Borland-Groover Clinic -- Patient Education -- MRCP vs. ERCP
MRCP correctly predicted the presence of CBD stones in 19 of 20 patients and failed to detect CBD stones in one patient with gallstone pancreatitis.
MRCP is an accurate, noninvasive test for evaluating the CBD duct for the presence or absence of calculi in patients suspected of having CBD stones.
The main indication for MRCP study is represented by the evaluation of common bile duct obstruction, with the aim of assessing the presence of the obstruction (accuracy 85-100 %) and, subsequently, its level (accuracy 91-100 %) and its cause.

MRCP is particularly useful in the evaluation of patients with suspected gallstone pancreatitis and in patients with non-specific abdominal pain and normal liver-associated enzymes.
MRCP is useful in demonstrating variant ductal anatomy and congenital anomalies of the biliary tract and pancreatic duct such as pancreas divisum, choledochal cyst, annular pancreas, abnormal pancreaticobiliary junctions and aberrant bile ducts.
MRCP depicts the mural irregularities, strictures and biliary calculi that characterize PSC.

MUSC Digestive Disease Center - Hot Topics
MRCP is based on a heavily T2 weighted pulse sequence which shows stationary fluids, such as bile, to appear at high signal intensity whereas the surrounding liver and flowing blood generates little signal.
MRCP is very accurate in assessing biliary and pancreatic disease in children (13-16).
MRCP obviated ERCP by excluding choledocholithiasis in patients with acute pancreatitis (n=12) and nonspecific abdominal pain (n=82).

MRCP is a less costly, non-invasive, and sensitive technique for evaluating the biliary and pancreatic ductal systems.
In MRCP, multiplanar images are obtained parallel to the orientation of the biliary tree, using an MR sequence that is sensitive to static fluid without the need for exogenous contrast agents.
MRCP image shows dilatation of intrahepatic ducts along with stricture from the tumor seen at the confluence of right and left intrahepatic ducts (arrow).

MRI - MR Body Applications ~ Current Capabilities   (Site not responding. Last check: 2007-11-03)
Typically, MRCP is performed using imaging parameters that highlight fluid as the main signal source for enhanced visualization of the biliary tree structures.
The major disadvantage of the fluid-only, long-TE MRCP images is that periductal structures are not well evaluated on the final images and may be of equal importance as intraluminal contents.
An alternative approach to long-TE MRCP is to use an intermediate TE in the range TE = 100- 300 ms (Figure 3).

The diagnostic performance of MRCP is somewhat lower for detection of malignancy with 88% sensitivity (95% CI: 70–96%) and 95% specificity (95% CI: 82–99%).
Congenital variants in biliary ductal anatomy may be demonstrated on MRCP as well with one series of 171 subjects reporting 86% sensitivity and 100% specificity for detection of variant cystic duct insertion.
Diagnosis of biliary disorders such as sclerosing cholangitis may be difficult on MRCP because of its relatively low spatial resolution and because the morphologic changes of this disorder may be subtle; however, one study reported that MRCP detected the disease with 85–88% sensitivity and 92–97% specificity.

BioMed Central | Full text | MRCP compared to diagnostic ERCP for diagnosis when biliary obstruction is suspected: a ...
MRCP is a comparable diagnostic investigation in comparison to ERCP for diagnosing biliary obstruction.
Indications for the use of MRCP include: unsuccessful or contraindicated ERCP; patient preference for non-invasive imaging; patients considered to be at low risk of having pancreatic or biliary disease; patients where the need for therapeutic ERCP is considered unlikely; and those with a suspected neoplastic cause for pancreatic or biliary obstruction [4].
The main advantage of MRCP is that diagnostic ERCP may be associated with significant morbidity and mortality [39].

 MRCP
MR Cholangiopancreatography (MRCP) is a method of depicting the biliary tree and the pancreatic duct non-invasively and without the injection of contrast medium.
The study can be acquired during quiet breathing, which improves the versatility of the technique because patients can be imaged regardless of their ability to breath-hold.
Today, MRCP represents a new development in MR technology that provides a non-invasive accurate means of evaluation of the biliary tree and pancreatic duct.
