There are 25 major diagnostic categories in all, most of them based on organ systems.
MDCs and DRGs were originally developed at the Yale University School of Organization and Management during the 1970s under contract to the Health Care Financing Administration.
The creation of a clinically coherent set of major diagnostic categories was the first step in the process of creating diagnosis-related groups.

MDC 15, Newborn and other neonates with conditions originating in the perinatal period, had a number of DRG revisions proposed, but these were not implemented in the final rule.
Category 633, Ectopic pregnancy, had subcategories expanded to five digits to enable coding a multiply gestation pregnancy that is both an ectopic and intrauterine pregnancy (633.00-633.91).

A final source of confusion is that major diagnostic category (MDC) 15 does not include all admissions of patients under 28 days of age, so a small percentage of newborns are scattered across many DRGs outside of MDC 15.
A major difference between PMDRGs and DRGs is that the former includes all non-normal newborns within the neonatal major diagnostic category (MDC 15).
By expanding the number of DRG categories from 7 to 31, as in the PMDRG model (without the mechanical ventilation groups), or even to 18, as in the BWDIS model, the number of categories could create a problem of small cell size.

The major surgery variable was flagged if the patient underwent one or more procedures that are generally intended as therapeutic (not diagnostic) and that must typically be performed in the sterile environment of an operating room or comparable facility.
The diagnostic evaluation variable was also flagged if the proportion of total charges accounted for by radiology plus laboratory charges was at or higher than the 75th percentile for the entire distribution of discharges for that cancer.
In addition, the diagnostic technologies employed for colon and breast cancers (e.g., radiographic barium studies of the colon, colonoscopy, needle biopsies of the breast) are generally performed on an outpatient basis; an admission purely for diagnostic evaluation would be relatively uncommon for these cancers.

All admissions are classified by major diagnostic category (MDC) and then subassigned to one of 470 diagnosis-related groups (DRGs) using the DRG grouper current at the time of discharge(1) as provided by Health System International/3M.
The remaining categories were based on physiologically meaningful intervals specific to each of the seven tests under study.
Consider, for example, the categories we used for "minimum albumin": category 0 is "not ordered," category 1 is "greater than or equal to an adjusted (see further on) lower limit of normal," categories 2 through 5 are "abnormal" (minimum) values falling in the ranges 2.9-3.1, 2.6-2.9, 2.3-2.6, and less than 2.3, respectively.

 MDC Report
The report, called Disability Benchmarks by Major Diagnostic Category, was prepared by Work Loss Data Institute (WLDI) to answer the question "How are we doing?", which is increasingly being asked by employers, disability insurers, and consultants.
To prepare the report WLDI mapped the raw ICD9 data in these databases to the 25 Major Diagnostic Categories (MDC) which are commonly used in benchmarking.
Numbers are summarized for all MDC’s showing that total lost-work days, for any unscheduled absence due to illness or injury (including sick leave, short and long term disability, and workers’ compensation), add up to 850 days per 100 workers (or 8.5 days per worker, equating to an absence rate of 3.4% assuming 250 work-days annually).

Although cost caps and carve outs are not personal attributes that can predict costs, when they are part of the capitation payment model, as the 1996 study by Weiner and others aptly demonstrates, they increase the explanatory power of the risk predictor statistic.
EDSs were then collapsed to risk-adjustment categories (RAdCs), such as healthy, moderate acute, single chronic, multiple chronic, three or more dominant chronics, metastatic malignancies, and catastrophic illnesses or conditions.
Even if the HCC variations are excluded, DCGs have evolved adjusted categories since their creation, so in the purest sense the DCGs reviewed in Table 3 are similar, but not equal.

A third approach is the Diagnostic Cost Group (DCG) methodology developed by Ash and her colleagues (Ash et al., 1986, 1989) to analyze Medicare data; the DCG approach has since given rise to several variants, such as the Hierarchical Coexisting Conditions model (Ellis et al., 1996), which takes into account multiple coexisting conditions.
Some of the categories are based on body systems and others on types of illness or disability, and the diagnostic categories fit into two broad classes.
The central feature of the ACG mechanism is a set of 32 distinct Ambulatory Diagnostic Groups (ADGs), which incorporate a majority of the ICD-9 diagnostic codes used in medical records (see Technical Note).
