It is slightly convex in front and concave behind; broad above, becoming narrowed at the point where the manubrium joins the body, after which it again widens a little to below the middle of the body, and then narrows to its lower extremity.
The superior border is oval and articulates with the manubrium, the junction of the two forming the sternal angle (angulus Ludovici 19).
The sternum is composed of highly vascular cancellous tissue, covered by a thin layer of compact bone which is thickest in the manubrium between the articular facets for the clavicles.

The manubrium is united to the body of the sternum either by an amphiarthrodial joint—a piece of fibrocartilage connecting the segments—or by a diarthrodial joint, in which the articular surface of each bone is clothed with a lamina of cartilage.
The first pair of ribs with the manubrium sterni move as a single piece, the anterior portion being elevated by rotatory movements at the vertebral extremities.
The thrust of the anterior extremities carries forward and upward the body of the sternum, which moves on the joint between it and the manubrium, and thus the antero-posterior thoracic diameter is increased.

The coupling between the eardrum and the manubrium is a case of impedance matching, analogous to the impedance matching required between the air in the ear canal and the liquid in the cochlea.
The fact that the manubrium is tapered may be seen as an attempt to avoid an abrupt impedance change, by gradually decreasing the stiffness from the thick and rigid head and neck of the malleus towards the centre of the relatively flexible eardrum.
The motion of the manubrium is determined by the combined effects of (1) the forces exerted on it by the eardrum; (2) the forces exerted on it by the ossicular and cochlear loads; (3) its own inertial and damping forces; and (4) the acoustical forces acting directly on it, which are small.

The mechanical coupling between the eardrum and the manubrium was studied by means of a finite-element model of the cat eardrum.
The lever action was generally considered to have two components: (a) the ratio of the eardrum area to that of the oval window; and, less importantly, (b) the ratio of the lever arm of the manubrium to that of the long process of the incus.
The fact that there is not a simple rigid coupling of the drum to the manubrium is further attested by the fact that the nodes in the region close to the manubrium have considerably smaller coupling values than those on the manubrium.

The inferior tip of the manubrium is located at the umbo of the eardrum.
