The road which runs in 3 lanes in both directions, is part of the London Inner Ring Road and as such forms part of the boundary of the zone within which the London congestion charge applies.
Construction of the New Road, as it was called, began in 1756 along the northern edge of the built-up area.
As part of the Ring Road and a feeder route to the A40 (and hence the M40 motorway) (to the west) and the A5 and M1 motorway (to the north) much of the traffic leaving central London for the Midlands and the North of England travels on this road.

Marylebone is roughly defined as the area north of Oxford Street, south of Regent's Park, east of Edgware Road and west of Portland Place (or alternatively Great Portland Street).
Marylebone gets its name from a church called "St Mary's" (now known as St Marylebone Parish Church) which was built on the bank of a small stream or "bourne", called the Tybourne, in an area named after the stream, Tyburn.
The Metropolitan Borough of St Marylebone was a metropolitan borough of the County of London between 1899 and 1965 after which it was merged with the Metropolitan Borough of Paddington and the Metropolitan Borough of Westminster to form the London Borough of Westminster.

You can actually trace the path of the road through Middlesex very easily as the North Orbital Road is still named as such (try looking it up in the index of a London A-Z or on Streetmap).
It is unclear exactly where the road would have gone from here (it's beyond the boundary of the diagram), but the A414 has long been an unofficial north-of-London bypass and indeed it has primary status.
