Topic: Mastocytoma

In the News (Tue 29 Dec 09)

  Indian Pediatrics - Editorial
Mastocytoma usually appear as solitary lesion, although it is estimated that 10% to 15% of all patients with one of the mast cell disorders (urticaria pigmentosa, telangiectasia macularis eruptive perstans, diffuse mastocytosis, systemic mastocytosis, and mast cell leukemia) have a mastocytoma.
Mastocytomas have a smooth or peau d’orange surface with a rubbery consistency.
Mastocytomas may precede another form of cutaneous mast cell disease or be associated with attacks of generalized flushing and abdominal colic.

 Pediatric Mastocytosis
In the child with isolated mastocytomas with resultant severe systemic symptoms from mast cell mediators, surgical excision of the mastocytoma may be the treatment of choice, although the majority of mastocytomas spontaneously resolve after several years.
Mastocytoma, the other common form of mastocytosis presents in 10 to 15 percent of children with mast cell disease.
Diffuse cutaneous mastocytoma is a rare form of the condition that is characterized by diffuse infiltration of the entire skin by mast cells.

We report a case of solitary mastocytoma in a child which was successfully treated with a topical steroid of moderate potency.
Solitary mastocytoma is a condition that occurs in infants characterized by localized mast cell hyperplasia and release of their mediators.
Spontaneous total regression is the natural course in 25% of mastocytoma cases; in another 25% the regression is partial, which indicates that treatment is not often recommended (5,6).

The term mastocytoma has used to described nodular infiltrates (as in Quiz case) of mast cells occurring single or one of several isolated lesions.
Solitary mastocytomas occur almost exclusively in the first 2 years of life and are often present at birth.
Therefore are self-limited and not-threatening, reassurance of the usual benign course of this disease and avoidance of specific factors known to trigger mast cell degranulation such as immunologic (allergens), physical (heat, cold, sunlight, trauma), biologic toxins (snake, insect, jellyfish and shellfish) and drugs (aspirin, alcohol, narcotics).

Mastocytoma is the name given to a localised form of mastocytosis.
Mastocytoma usually appears in early childhood at a few months of age.
If the mastocytoma is causing a lot of itching, this can often be relieved with oral antihistamines.

Mastocytomas are often associated with an infiltration of inflammatory cells which can contain neutrophils and sometimes eosinophils.
When a mastocytoma is diagnosed from cytology and aggressive excision of the tumour, with wide margins is indicated and the tumour should be submitted for histopathology to determine its grade and evaluate if the excision was adequate.
It is also important to evaluate local lymphatic ganglia and pulmonary radiographs in order to check for the presence of metastases before deciding if the patient is a surgical candidate.

Expression of the Mouse Mastocytoma Glucosaminyl N-Deacetylase/N-Sulfotransferase in Human Kidney 293 Cells Results in ...
Proteins expressing both activities have previously been purified from mouse mastocytoma, which generates heparin, and from rat liver, which produces heparan sulfate.
In the present study, the mouse mastocytoma enzyme has been expressed in the human kidney cell line, 293, to investigate whether it could promote modification of the endogenous heparan sulfate precursor polysaccharide into a heparin-like molecule.
The N-deacetylase activity of the stably transfected cell clones was ~8-fold higher, on a cell-protein basis, than that of control cells, while the N-sulfotransferase activity was increased ~2.5-fold.

Mastocytomas are the skin lesions that are seen in people with urticaria pigmentosa.
The release of this histamine causes the welting, hives, flushing, and headache associated with urticaria pigmentosa.
Urticaria pigmentosa is characterized by few to many brownish spots which itch and, when scratched, produce welts and reddened skin.

The Mastocytosis Society: Research - Featured Articles - A Consensus Document
In skin, accumulation of groups of mast cells combined with the presence of urticaria pigmentosa or mastocytoma is diagnostic of cutaneous mastocytosis.
However, elevated total tryptase levels occur temporarily after extensive mast cell mediator release and have been seen persistently in some other diseases such as non-mast cell leukemic and pre-leukemic blood disorders, so when one of these disorders is present, the level of total serum tryptase cannot be regarded as a diagnostic marker for systemic mastocytosis.
People with extra cutaneous mastocytoma do not fit the criteria for systemic mastocytosis, and they have no skin lesions, and pathology examination of the lesion shows it to be made up of normal- or nearly normal-appearing mast cells with a non-aggressive growth pattern.

eMedicine - Mastocytosis : Article Excerpt by: Daniel Hogan, MD
Background: Mastocytosis is a disorder characterized by mast cell proliferation and accumulation within various organs, most commonly the skin.
Types of cutaneous mastocytosis include solitary mastocytoma (M), diffuse erythrodermic mastocytosis, paucicellular mastocytosis (also termed telangiectasia macularis eruptiva perstans [TMEP]), and urticaria pigmentosa (UP).
UP is the most common form and is characterized by oval or round red-brown macules, papules, or plaques ranging in number from a few to thousands (see Images 1-2).
