McDowell County is considered as one of the core counties in Appalachia on which the national War on Poverty focused, a national effort started during President Lyndon B. Johnson's administration.
McDowell County is bounded in the northwest is Mingo County, West Virginia, in the north by Wyoming County, West Virginia, in the east Mercer County, West Virginia, in the south by Tazewell County, Virginia, and in the west by Buchanan County, Virginia
In the county, the population is spread out with 23.20% under the age of 18, 7.90% from 18 to 24, 26.80% from 25 to 44, 26.10% from 45 to 64, and 16.10% who are 65 years of age or older.

In McDowell County, water and sewer infrastructure is being developed in a process that is largely top-down; in Letcher County, planning and implementation has proceeded on the grassroots level and combines concepts of both regionalization of infrastructure and of locally-tailored solutions.
Similarly, McDowell County has declined from nearly 100,000 persons fifty years ago to about 30,000 today. A declining population means a declining tax base, particularly when a lack of financial resources in the population discourages and investment in maintenance of existing structures, both commercial and residential, let alone new business ventures and new construction.
In McDowell County, West Virginia, where water and sewer planning and implementation follows the traditional top-down model, the goals of safe drinking water and proper sewage treatment for all citizens are the same as in Letcher County, where planning sprang from a grassroots assessment of local needs and resources.

McDowell County was created by an act of the Virginia General Assembly on February 28, 1858, from parts of Tazewell County (Virginia).
The son of Irish immigrants, he served as a member of the Virginia Assembly (1831-1843), was the 25th Governor of Virginia (1843-1846), and represented Virginia in U.S. House of Representatives from 1846 until his death in 1851.
The act creating the county specified that the county seat was to be located at Perryville, and the initial meetings of the county court where held there, in the home of George W. Payne.

A complaint was filed under title II of the Americans with Disabilities Act (ADA), 42 U.S.C. §§ 12131-12134, with the Department of Justice against the County, alleging, inter alia, that the McDowell County Commissioner's Office (the "Commissioner's Office"), Sheriff's Department, Courthouse Annex, and County Courthouse (the "Courthouse") are inaccessible to individuals with mobility impairments.
The ADA applies to the County because it is a public entity as defined in the Department of Justice's regulation implementing title II of the ADA.
The County agrees that the various services, programs, and activities that it provides to the public at the Commissioner's Office, Sheriff's Department, Courthouse Annex, and Courthouse shall be made accessible to all qualified individuals with disabilities.

The economy and population of McDowell County, West Virginia, drastically decreased between 1950 and 1970.
Using production and employment data of two representative McDowell County coal companies, it is clear that the introduction of continuous mining machines, which combined the cutting and loading of coal into one step, allowed companies to mine more coal with fewer workers.
Because the economy of McDowell County was so coal-intensive, the increased unemployment caused by mechanization forced many miners to migrate to such midwestern industrial centers as Cleveland or Columbus.

Welch, the county seat, is the largest city in McDowell, with the population of just over 3,000.
It is magnificently timbered and today still appears to have coal deposits under the mountainous acres of land in the county.

The Rural School and Community Trust has recently collaborated with community organizers in McDowell County, West Virginia to produce McDowell County People's Plan for Excellence, an alternative plan to the school closures proposed by the State Department of Education.
In fall 2001, one month after the State Department of Education took over the McDowell County School system, the Department announced its plan to close two high schools, two middle schools, and nine elementary schools.
Citizens have collaborated under the leadership of a local group called Teachers, Parents, Students, and Others (TOPS) and with the support of Challenge West Virginia, a statewide nonprofit group, and numerous local organizations to produce an alternative to the closures.
