The way attachments are handled in MediaWiki make it, in my opinion, untenable for an intranet.
MediaWiki has one flat space for attachments, so that every one has to have a different name.
I think this is just a reflection of the fact that MediaWiki is designed for the wikimedia projects, where you have people working on a single collective project.
