Melancholics are characterized by the element of Earth, the season of Autumn, middle-aged adulthood, the color blue, and the characteristics of "Cold" and "Dry." Famous Melancholics include St. John of the Cross, St. John the Divine, St. Francis, and St. Catherine of Siena.
Melancholic persons who give way to this disposition to look at everything through a dark glass, gradually become pessimists, that is, persons who always expect a bad result; hypochondriacs, that is, persons who complain continually of insignificant ailments and constantly fear grave sickness; misanthropes, that is, persons who suffer from fear and hatred of men.
Melancholic educators, too, often commit the fault of keeping silent too long about a fault of their charges and when at last they are forced to speak, they do it in such an unfortunate and harsh manner, that the pupils become discouraged and frightened by such admonitions, instead of being encouraged and directed.

A melancholic person is made suspicious and reticent by a rude word or an unfriendly mien; by continuous kind treatment, on the contrary, he is made pliable, trusting, affectionate.
And if a melancholic person is slow, hesitating, undecided; if he does not speak much and even if he says awkwardly the little he has to say; or if a sanguine person is very talkative, light-minded, and frivolous; if a phlegmatic cannot be aroused from his usual indifference, he does not become irritated.
Many things which cause a melancholic person a great, deal of anxiety and trouble do not affect the sanguine in the least, because he is an optimist and as such overlooks difficulties and prefers to look at affairs from the sunny side.

Melancholic patients were significantly slower than controls with no reduction in external cues and with a moderate reduction in external cues.
At a high level of reduction in external cues, seven of 12 melancholic patients (but only three of 12 non-melancholic patients and controls) were unable to complete the task; suggesting a greater reliance on external cues, perhaps implicating a failure of motor planning ability in melancholic patients.

In the brain, low levels of the inhibitory transmitter GABA and high levels of the excitatory neurotransmitter glutamate appear to be strongly associated with a particular type of depression, according to a study by Yale researchers.
Author Gerard Sanacora, assistant professor of psychiatry at Yale School of Medicine and director of the Yale Depression Research Program, said the reductions in cortical GABA and increased glutamate were particularly associated with the presence of melancholic and psychotic features in patients with major depressive disorder.

Melancholic depressed patients who are 40 years or older, especially men, appear to show a superior response to the tricyclic antidepressant drug (TCA) nortriptyline, whereas younger patients, especially women, show a superior response to the serotonin reuptake inhibitor (SSRI) fluoxetine.
The researchers report that melancholic depressed patients aged 40 years or older, especially men, had a superior response to nortriptyline compared with fluoxetine based on CORE criteria.
Likewise, with DSM-IV criteria, the response rates of melancholic patients aged 40 years or older were 60% with nortriptyline and 53% with fluoxetine, while in the melancholic patients 18-24 years the response rates were 69% with fluoxetine and 44% with nortriptyline.
