Melanocytic nevus is a benign proliferation of melanocytes or melanocytic nevus cells that appear in childhood.
The nevus cells are thought to be derived from melanocytes which migrate to the epidermis from the neural crest during embryonic development.
Melanocytic nevi reach a peak in puberty and tend to decrease thereafter.

 Nevus
The mucosal nevus or "mole" is the same as the moles which occur so commonly on the skin, although it is decidedly rare in the mouth.
Although rare in the mouth, the cutaneous melanocytic nevus is probably the most common of all human tumors, with white adults averaging 10-40 nevi each.
The cutaneous melanocytic nevus is first seen during childhood and most lesions are present before 35 years of age; most oral examples are biopsied during the young adult or middle-age years.

Melanocytic nevus cell "rests" can be observed in the capsules of lymph nodes, and they can sometimes be mistaken for metastatic deposits because of their extracutaneous location.
Melanocytes in a nevus have lost their contact inhibition and are found in nests.
As the melanocytes proliferate, small nests of cells develop in the lower-most part of the epidermis, and the resultant lesion is termed a junctional melanocytic nevus.

Spitz nevi are acquired benign melanocytic lesions that occur in childhood and adolescence.
A distinctive variant of melanocytic growth pattern is described, which appears to be related to Spitz nevus and is characterized by a mainly intraepidermal proliferation of large epithelioid melanocytes with a predominantly pagetoid distribution.
Thus, although the distinction between benign and malignant melanocytic tumors could and should not be based on immunohistology for Ki-67 and p53, these results suggest that the latter determinants may, in fact, be used as an adjunct to morphology in the recognition of the vertical growth phase in cutaneous malignant melanomas.

The main melanocytic lesions in the genital area include the mucosal lentigo, the common acquired nevus, the dysplastic nevus, melanoma, and a distinctive form of nevus termed "atypical melanocytic nevus of the genital type" by Wallace Clark et al (1999).
The dermal component of the dysplastic nevus is usually banal in contrast with the large oval nuclei that may be seen in the superficial component of the milk line nevus.
The histology of LM is one of polygonal melanocytes with hyperchromatic, angulated nuclei dispersed as individual units, initially confined to the basal layer of epidermis in a discontiguous ashion and extending along the eccrine ducts and the outer root sheath epithelium of hair follicles.

Interest in melanocytic nevi stems from their clinical, histological and epidemiological association with malignant melanoma.  Numerous case-control studies have shown that the number of melanocytic nevi is the strongest phenotypic risk factor for melanoma (Armstrong and English 1992).
As the etiology of melanocytic nevi has been under investigation for a shorter period of time than melanoma, numerous questions remain to be answered (Armstrong and English 1988).  However, there are many epidemiologic observations linking solar UVR to the development of acquired melanocytic nevi.
Studies of prevalent melanocytic nevi, particularly those conducted in similar age groups, show a general tendency for nevus frequency to be highest in white populations living closest to the equator (Harrison et al 1999).  However, temporal and methodological differences may cloud the results of such broad comparisons.
