Microfracture surgery - Wikipedia, the free encyclopedia
Microfracture surgery is an orthopedic surgical technique used to restore damaged knee cartilage, the material that helps cushion bones at the joints.
Originally, the surgery was called "controversial" by many sportswriters, due to a lack of studies on the long-term effects and the fact that an unsuccessful surgery could end an athlete's career.
Because microfracture surgery is quick (taking around 30 minutes), less invasive than previous types of knee surgery, and has a high rate of success, it has quickly become a common procedure for orthopedic surgeons.

How microfracture is performed: A pick-like instrument is used to put holes roughly an eighth-of-an-inch deep into the exposed bone in the joint where the articular cartilage has worn away.

Medical Encyclopedia: Knee microfracture surgery (Print Version)
Microfracture is a common surgical technique used to repair damaged knee cartilage, the material that helps cushion bones at the joints.
The new fibrocartilage created by microfracture surgery is less durable than the body’s original cartilage, and can break down after a few years.
Physical therapy begins in the recovery room immediately after surgery.  The patient is connected to a continuous passive motion (CPM) machine that gently flexes and extends the leg for 6 to 8 hours per day for several weeks, often during sleep.

Kidd underwent microfracture surgery on his left knee July 1, and it's unclear whether the 31-year-old point guard will be ready to play by his projected December return or whether he will ever be the same.
Microfracture surgery generally requires a four-to-six month rehabilitation period for athletes, so Kidd theoretically could be back running the floor in December, as planned.
Each had microfracture surgery on the same day in June 2003, and neither was able to come back strong last season.

Microfracture for damaged knee cartilage - KNEE ARTHRITIS - Bone marrow stimulation
I recently read an article about Terrell Davis having a type of knee surgery where they drill holes into the bone and let the bones bleed leaving a scab which serves as a cushion between bones that serves in essense as replacement cartilage.
Microfracture is similar to, but not the same as, drilling.
The issue with microfracture is that the patient needs to be very careful to not fully weight bear for up to three months to give the joint surface enough time to really heal over with the new cells, and this seems to be where the failures occur, as people are often too impatient.

In a microfracture procedure, doctors use a picklike instrument to scoop out tiny dimples in the part of the bone that is close to the damaged cartilage.
Though the Suns had hoped a basic arthroscopic surgery would fix the problem and that Stoudemire would be back in a few weeks, Carter figured microfracture surgery would be necessary after he looked at the MRI.

The rehabilitation program after microfracture is crucial to optimize the success of the surgical technique.
When the patient wears a brace, strength training is allowed, but only in the 0° to 20° range immediately after surgery in order to limit compression of the affected chondral surfaces.
The joint angles of these patients are observed carefully at the time of surgery to determine where the defect makes contact with the opposing surface, either on the patella or on the trochlear groove of the femur.

Pioneered by Dr. Steadman in the mid-Eighties, microfracture involves the use of an arthroscopic shaver to remove cartilage remnants and surgical awls to make perforations through the subchondral bone plate, thus allowing access to stem cells and promoting important healing factors.
Without this revolutionary surgery, it’s likely that Wyman and Cauchois would be facing total knee replacement at some point, sooner rather than later, during their lives.
But what really turned his life around was microfracture, which Dr. Steadman performed on him in 1989, his second year in the pro leagues.
