Such volunteer service must be made in a sustained and direct nature towards the civilian community, must be significant in nature to produce tangible results, and must reflect favorably on the military service and the United States Department of Defense.
The definition of volunteer service is left intentionally vague, allowing for a wide variety of activities and volunteer duties which would qualify a service member for the Military Outstanding Volunteer Service Medal.

The Cold War Victory Medal was designed by Nadine Russell, the Chief of Creative Heraldry at the Army's Institute of Heraldry and the designer of many campaign and service medals, including the Southwest Asia Service Medal, the Armed Forces Service Medal, and the Military Outstanding Volunteer Service Medal.
However, the medal is worn as a National Guard award as issued by the Louisiana National Guard.

There is hereby established a Military Outstanding Volunteer Service Medal, with accompanying ribbons and appurtenances, for award by the Secretary of Defense or, with respect to the Coast Guard when it is not operating as a service in the Navy, the Secretary of Transportation.
No more than one award of the Military Outstanding Volunteer Service Medal may be made to any one person, but for each subsequent act justifying such an award, a suitable device may be awarded to be worn with that medal as prescribed by appropriate regulations issued by the Secretary of Defense.
The Military Outstanding Volunteer Service Medal may be awarded posthumously, and when so awarded, may be presented by such representatives of the deceased as may be deemed of the Coast Guard when it is not operating as a service in the Navy, the Secretary of Transportation.

Each service has its own version of this medal, but it is universally known to have a blue sash with white stars embrodiered in the material and the ribbon is a light blue with a field of white stars across it.
The Yangtze Service Medal was authorized in April 1930 for award to Navy and Marine Corps personnel who served in China as part of the Yangtze River Patrol between September 3, 1926 and October 21, 1927 or March 1, 1930 and December 31, 1932.
A campaign medal was designed for each Theater, and participation in designated battles or campaigns was designated by the use of small campaign stars (rather than bars as was the case for the WW I Victory Medal) affixed to the medal and service ribbons.
