 Patient dies after tube inserted into lungs instead of stomach, Japan
An 84-year-old patient died in a hospital in Matsudamachi, Kanagawa Prefecture, Japan, after a nurse mistakenly inserted a nutrient tube into his lungs instead of his stomach, prefectural authorities said Monday.
The incident occurred at about noon Thursday, and the man, from Minamiashigara, died Friday night at the Ashigarakami Hospital, run by the Kanagawa prefectural government.
The nurse had attempted to push the tube back into the man's stomach through the nose after pulling it out because it was loose, they said.
