  Encyclopedia of Arda: Morannon
It seems reasonable to consider the Morannon as part of this fortification, which would mean that it stood for most of the Second Age, as shown in the timeline above.
At the very least, we know it was in place by the end of that Age, because it is mentioned in accounts of the War of the Last Alliance.
It is not known what happened to the Morannon during the Third Age, before its final destruction in the War of the Ring.

Effective distance is based on real distance and existence of a road (maybe complete or only partial).
For example, real distance between Barad-Dur and Morannon is 150 miles, there is a road, so effective distance is only 100 miles.
From the other hand, distance between Barad-Dur and Frijar nomad camp is 345 miles, and there's only 90 miles of road (Nurnen Road), thus only 30 miles of effective distance are spared.

09-19-03: Crowning of Elessar Before Battle of the Morannon?
Oh yes, here we go with this interesting bit of educated speculation (hmm...I don't even know if that sentence works).
There’s also a shot of him in the DVD preview approaching the Throne of the White Tower in his RANGER clothes, and I can’t see why he’d do this without the intention of claiming his kingship (he does nothing of the sort in the book prior to his crowning).
Now some may say Aragorn simply changes his clothing before setting out for the Morannon, and that is still a possible alternative to this theory, and I can't deny it.
