Moricizine is a phenothiazine derivative with Vaughan Williams class 1 antiarrhythmic properties.
Moricizine is extensively metabolized and may have pharmacologically active metabolites.
Further studies are needed comparing moricizine with other class 1 agents in the treatment of life-threatening arrhythmias; available data suggest that moricizine is comparable with these agents in the treatment of ventricular tachycardias and fibrillation.

In April of 1989, the Data and Safety Monitoring Board recommended that the encainide and flecainide arms of the study be discontinued because of excessive mortality in the drug arms of the trial primarily due to arrhythmia, acute MI with shock, or chronic congestive heart failure.
CAST-II was stopped early because long-term treatment with moricizine after an MI was associated with a trend to excess mortality as compared with no treatment or placebo.

Prior to actual randomization, there was an open-label titration period to identify patients who responded to treatment.
A total of 1,727 patients who responded were randomized: 1,455 to encainide, flecainide, or placebo, and 272 to moricizine or placebo.
CAST continued to compare moricizine to placebo in 1,300 patients for 18 months until August 1991 when the moricizine portion of the trial was stopped early because of excess deaths.

At the completion of the long-term phase, there were 49 deaths or cardiac arrests due to arrhythmias in patients assigned to moricizine, and 42 in patients assigned to placebo (adjusted P = 0.40).
As with the antiarrhythmic agents used in CAST-I (flecainide and encainide), the use of moricizine in CAST-II to suppress asymptomatic or mildly symptomatic ventricular premature depolarizations to try to reduce mortality after myocardial infarction is not only ineffective but also harmful.
