While the city of Mournhold appears to be similar to the open-air towns of the original game, Mournhold is actually akin to an interior room.
Players cannot levitate while in Mournhold, because levitation would reveal that the "sky" of Mournhold is little more than a ceiling.

Traveling to Mournhold is a bit of a pain, as you'll need to find a specific person who can magically teleport you there.
The general gameplay in Mournhold follows that of the rest of Morrowind to a tee, which I have no real complaints about.

Tienus Delitian is important to Mournhold's main quest.
You can't teleport to Mournhold before talking to the guards, because talking to the guards starts the quest, and gives you a lead to follow.

Mournhold is split up into 5 different sections, The Plaza, which connects you to the other sections, Godsreach; which is were most of the citizens of Mournhold live, Great Bazzar; which contains many shops and attractions, the Temple; where the giant temple complex is located, and the Palace; where King Helseth lives.
This is a pic of the Mournhold Plaza.
Mournhold is probably the most beautiful city in the game.

There is no set requirement for when the player must travel to Mournhold, and they are, in fact, free to come and go as they please.
But comparisons between the two notwithstanding, the quests to be found in the expansion were of such a high quality that any disappointment to be found in the size of the world or in the linearity of the story are more than made up for.
For example, if you discover a Nord residing in the sewers of Mournhold but are unsure as to what you should do with him, you can mark him on your map for easy returning later.

Mournhold is fairly large-seeming (although for a capital city it really isn't that large) - it consists of five surface regions - the Temple, the Palace, the Plaza Bridisi Boron, the Great Bazaar and Godsreach.
The remainder of Mournhold houses shops, inns, residences and even a small theater; many of the houses and shops have their own subquests, and even some passersby will bemoan their fate and plead your assistance.
To get into the real meat of Mournhold, you'll have to visit either ruler of the city; you are prompted to go there from the start, but you can really visit either the sacred (the Goddess Almalexia of the Tribunal) or the secular (King Helseth of Morrowind).

With the advent of the Tribunal expansion, you get to travel to the capital city of Mournhold, through which you will meet the remaining two: Almalexia, and Sotha Sil, both of whom have issues and motives of their own that draw you in and threaten to destroy the land.
There are also a number of brand new monster types scattered beneath the city of Mournhold, such as liches, goblins, fabricants, and more; while the goblins and clockwork beasts are particularly nicely modeled and animated, the rest leave much to be desired.
Not all of these minor additions are a bust, however, as Mournhold's Museum of Artifacts is a wonderful touch that breathes new life into your pursuits of strange and powerful items.

For one thing - and this actually came as something of a surprise to me - 'Mournhold' was not actually the name of the entire city I stood in (even though, confusingly, many people still referred to Mournhold as a city): it was the name of the holy district at the centre of that city.
This was why everything in Mournhold was so massive and majestic: the whole district had been set aside and walled-off in order to better maintain it as a pristine and suitably striking dedication to the goddess Almalexia.
At the time of my visit, passage between Mournhold and the rest of the city was prohibited; and the reason for this did not surprise me: there was concern over the curiously high number of people teleporting into the district from Vvardenfell when there was supposedly a quarantine in effect.

Mournhold is actually upon the mainland, to the SE of Vvardenfell, but you can't get there without teleporting.
The capital of Morrowind is called Almalexia, and Mournhold (Or the Mourning Hold, as I've heard it referred to) is in the middle of it.
As Pixel_Toa described it, Mournhold is the Vatican of Almalexia.

Actually getting to Mournhold requires a ridiculous expenditure of effort every time—the only point of access is through the fortress of Ebonheart, and a less conveniently placed locale to make your base of operations there never has been—then getting back can also be a chore.
It's just as good-looking as ever, and I was pleased to see that designers flexed their creative muscles by making Mournhold and surrounding regions visually different from Vvardenfell without making it seem like you were in another country or something.

You should keep in mind that Mournhold is in the city of of Almalexia, but that it is a city state in its own right.

Mysterious attacks are unleashed on Mournhold and ancient ruins that have been hidden for hundreds of years are exposed, and you must investigate these incidents and find out who's behind them and who you should trust: The Temple or The Empire.
Despite the huge underground areas, Mournhold as a City is actually pretty small, and I can only assume suburbs sprawl from outside the main gates.
Apart from the references to events in Morrowind, Mournhold is its own community with no Houses to follow or Guilds to join, making it almost completely detached from Vvanderfell.
