STD Treatment to Prevent HIV Infection
The first trial, conducted in Mwanza, a rural area of Tanzania, demonstrated a decrease of about 40% in new, heterosexually transmitted HIV infections in communities with continuous access to improved treatment of symptomatic STDs, as compared to communities with minimal STD services, where incidence remained about the same (Grosskurth et al., 1995).
In Mwanza, STD treatment was only for symptomatic STDs, whereas the mass treatment approach in Rakai treated both symptomatic and asymptomatic STDs.
Mwanza was experiencing a relatively early HIV epidemic, with community HIV prevalence of about 4%, whereas Rakai represented one of the world's most mature epidemics, with a community HIV prevalence of approximately 16%.

Preventing STDs and HIV Infection -- What are the lessons from Mwanza and Rakai   (Site not responding. Last check: 2007-09-30)
At the time of the study, the HIV epidemic was in its early stages in Mwanza -- the HIV sero-prevalence in the study population was estimated to be 4%.
Arguably, the Mwanza study reflects the short-term impact of STD prevention and control in an immature epidemic and the Rakai study reflects the short-term impact of STD prevention and control in a mature epidemic.
An alternative explanation is that the impact of STD prevention and control in Mwanza reflects differences in biology of the clad(s) in the regional epidemics -- if the Mwanza viruses were harder to transmit through vaginal intercourse, then treating STDs would have a greater effect.
