Mydriasis - Wikipedia, the free encyclopedia
There are two types of muscle which control the size of the iris: circular and radial.
A mydriatic is an agent which induces dilation of the pupil.
Drugs such as tropicamide are used in medicine to permit examination of the retina and other deep structures of the eye, and also to reduce painful ciliary muscle spasm (see cycloplegia).

 Mydriatic eyedrops and cataracts
Mydriatic eyedrops enlarge (dilate) the pupils of the eyes and may allow the person to see around a cataract.
Mydriatic drops are rarely prescribed for cataracts and are only used for a short period of time.
Mydriatic eyedrops may be used in infants and children under age 2 to enlarge the pupil, allowing more light to pass around a cataract that covers only part of the lens.

 Mydriatic-cycloplegic drugs and corticosteroids
Mydriatics and cycloplegics are used mostly in ophthalmic examinations but also for treating certain eye conditions.
Cholinergic agents, such as pilocarpine, are not normally used to reverse the effects of mydriatic drugs used for eye examination because of the discomfort the cholinergic agents cause through strong miosis and ciliary spasm.
However, shorter-acting mydriatics, such as homatropine or cyclopentolate, because of their short duration of action, allow some recovery from mydriasis giving the pupil mobility and consequently also reduce the risk of developing posterior synechiae.

Effect of mydriatic and cycloplegic drugs in glaucomatous and nonglaucomatous eyes using ultrasound biomicroscopy
The objective of this study is to evaluate, by means of ultrasound biomicroscopy, the anterior segment of the eye of the patient with chronic simple glaucoma and nonglaucomatous eyes after mydriatic and cycloplegic instillation.
An increase in the anterior chamber depth was observed by the UBM technique after the use of mydriatics and cycloplegics in groups G and NG, corroborating the findings of the literature 8.
Regarding the iris-lens contact, the response to the use of mydriatic and cycloplegic drugs was greater after the use of 1% tropicamide plus 10% phenylephrine chlorydrate in both groups (C2).

Journal of Pediatric Ophthalmology and Strabismus
All three mydriatic regimens provided adequate pupillary dilation at 45 minutes, with dilation sustained at 60 minutes.
Although there was no significant change of abdominal girth in any of the three groups, a total of eight patients developed intolerance to feeds; four (50%) of these infants were from the cyclopentolate 1% + phenylephrine 2.5% group.
The prepared combination of cyclopentolate 0.2% + phenylephrine 1% appears to be the mydriatic of choice for preterm infants with dark irides as it provided adequate pupillary dilation with the least systemic side effects.

News - Ibopamine More Potent Mydriatic Than Phenylephrine Or Tropicamide   (Site not responding. Last check: 2007-11-01)
Ibopamine alone has a more potent mydriatic effect and increase in intraocular pressure in patients with glaucoma than do either phenylephrine or tropicamide.
Researchers from the University of Verona, Italy, enrolled 15 patients with primary open-angle glaucoma and 15 healthy subjects, aged between 40 and 70 years (mean 54.8), into this open prospective study.
The authors concluded that 2% ibopamine is a more potent mydriatic than either 10% phenylephrine or 1% tropicamide.

et al concluded that, based on an assessment of available cohort studies, the most effective strategy for testing is the use of mydriatic retinal photography with the additional use of ophthalmoscopy for cases where photographs are ungradeable.
demonstrated that for mydriatic digital photography, in 1549 patients, for the detection of sight-threatening retinopathy, the sensitivity was 87.8%, specificity was 86.1% and ungradable image rate was 3.7%.

To establish whether an experienced endocrinologist could screen accurately for diabetic retinopathy using mydriatic 60° fundus photographs compared with a reference standard, viz.
The screening method used gave a sensitivity of 83% and specificity of 99% which are within recommended standards.
The screening strategy using a mydriatic fundus camera at the diabetic clinic was found to be effective and accurate and greatly reduced the number of possible referrals to the ophthalmology outpatient department.
