Myocarditis - Wikipedia, the free encyclopedia
In medicine (cardiology), myocarditis is inflammation of the myocardium, the muscular part of the heart.
Since myocarditis is often due to a viral illness, many patients give a history of symptoms consistent with a recent viral infection, including fever, joint pains, and easy fatigueability.
Myocarditis is often associated with pericarditis, and many patients present with signs and symptoms that suggest concurrent myocarditis and pericarditis.

Myocarditis may be a complication during or after various viral, bacterial, or parasitic infectious diseases, such as polio, influenza, rubella, or rheumatic fever.
It is often caused by various diseases such as syphilis, goiter, endocarditis, or hypertension, however, myocarditis may appear as a primary disease in adults or as a degenerative disease of old age.

E-Journal - Volume 1 - Virus induced myocarditis and inflammatory cardiomyopathy
Myocarditis and inflammatory cardiomyopathy are frequently caused by viral infections of the myocardium.
Myocarditis is an inflammatory disease of the cardiac muscle that is caused by intramyocardial infiltration by immunocompetent cells 1.
Chronic myocarditis and inflammatory cardiomyopathy, on the other hand, are both characterised by the immunohistologic proof of an active inflammatory but no virus is detected in the molecular biological analysis (autoimmune type).

Most cases of myocarditis in the United States originate from a virus, and the disease may remain undiagnosed by doctors due to its general lack of initial symptoms.
Myocarditis usually results from the Coxsackie B virus, and may also result from measles, influenza, chicken pox, hepatitis virus, or the adenovirus in children.
The best way to diagnose myocarditis may be through a person's observation of his or her own symptoms, followed by a thorough medical history and physical exam conducted by a doctor.

Acute myocarditis is a rare inflammatory disease characterized by lymphocytic infiltration and myocyte necrosis, which may lead to the sudden onset of cardiac failure.
Myocarditis can be caused by a variety of pathogens including bacteria, viruses, and parasites.
The clinical diagnosis of myocarditis is difficult and many times is arrived at only after more common pediatric problems including asthma, recurrent vomiting, and chronic viral illness such as mononucleosis are excluded.

 Myocarditis - Patient UK
So, myocarditis may develop at the same time as, or just after, a viral throat or chest infection, or when you have flu.
Viral myocarditis often occurs at the same time as viral pericarditis (inflammation of the sac-like tissue which surrounds the heart.
Other viruses which sometimes cause myocarditis include: echoviruses, influenza (flu), Epstein-Barr (glandular fever virus), rubella (german measles virus), varicella (chickenpox virus), mumps, measles, parvoviruses, yellow fever, dengue, polio, rabies, and the viruses that cause hepatitis A and C. Sometimes the inflammation in the heart lasts longer than other features of the infection.

Myocarditis is described as "an inflammatory infiltrate of the myocardium with necrosis and/or degeneration of adjacent myocytes" (Dallas classification, 1987).
Chronic phase: Continuing myocyte destruction is autoimmune in nature, with associated abnormal expression of human leukocyte antigen (HLA) in myocytes (and in the case of viral myocarditis, persistence of viral genome in myocardium).

We report a case of hypersensitivity myocarditis secondary to clozapine administration that was diagnosed in vivo for the first time through endomyocardial biopsy and was successfully treated with corticosteroids.
The histologic diagnosis was based on the evidence of eosinophilic infiltration of the endomyocardium, and on eosinophil degranulation with the likely release of the cationic protein that ultimately mediates both myocyte damage and endocardial thrombus formation.
Clozapine-induced hypersensitivity myocarditis should be suspected by psychiatrists and physicians when cardiac dysfunction appears suddenly, and appropriate diagnostic and therapeutic strategies must be undertaken promptly.

 Viral Myocarditis: Coxsackie Virus Infections
The coxsackie viruses are divided into two major subgroups, labeled A and B. There are 23 known coxsackie A viruses that usually cause only enteric diseases, and 6 known coxsackie B viruses, which are the ones of greatest concern because of their ability to cause serious diseases beyond the intestinal tract.
Effective medical therapies have yet to be worked out for viral myocarditis; one approach is to administer anti-inflammatory or immunosuppressive drugs during the early stage of the disease to impair the immune attack and resulting inflammatory response that damages the heart.
The main strategies are to inhibit viruses with the heat clearing herbs, of which sophora root (kushen) appears to be the most promising agent, and to bolster the resistance to the disease and limit its damaging effects to the heart with qi tonics, for which astragalus (huangqi) and ginseng (renshen) appear particularly valuable.

Myocarditis is an elusive illness to study, diagnose, and treat as the clinical presentation may range from nearly asymptomatic to overt heart failure requiring transplantation; a myriad of causes exist, and it is occasionally the unrecognized culprit in cases of sudden death.
Animal models of viral myocarditis have lead to a much greater understanding of the pathophysiology of acute, severe myocarditis and correlate with the findings in susceptible patients who apparently uptake viral RNA and develop a cytotoxic necrosis and rapid (1-2 d) cell death without the appearance of the interstitial infiltrate usually associated with myocarditis.
Myocarditis may present subtly, but it should be considered in the patient who presents with dyspnea and chest discomfort, particularly if the history includes a recent viral illness.

In the Myocarditis Treatment Trial, the 1-year mortality rate was 20% and the 4-year mortality rate was 56% in a population with symptomatic heart failure presentation and left ventricular ejection fraction lower than 45% at baseline.
In general, treatment of both acute and chronic myocarditis is aimed at reducing congestion and improving cardiac hemodynamics in heart failure, as well as providing supportive therapy with the hopes of prolonging survival (see Treatment).
In a study of 147 cases of myocarditis monitored for an average of 5.6 years, 93% of the 15 patients with fulminant disease were alive without transplant 11 years after biopsy, compared with 45% of the 132 patients with less severe disease.
