Genetic amplification (NAAT) test detects HIV more effectively than standard tests in urban study
Physician/researchers at Emory, the University of North Carolina at Chapel Hill and the Georgia Department of Human Resources used NAAT to screen clients receiving routing HIV testing and counseling at the urban clinics between October 2002 and January 2004.
NAAT testing has been used in the U.S. as an investigational screening test for donated blood since 1999 and was approved for use in 2002 by the FDA.
According to a study by the National Institutes of Health (New England J. of Medicine, Aug. 19, 2000), the test has helped prevent the transmission of approximately five HIV-1 infections and 56 hepatitis C virus infections each year since it was initiated for screening donor blood.

NAAT findings were positive in specimens from 12 of 14 patients with extrapulmonary tuberculosis and in 0 of 18 cases in which tuberculosis was excluded (sensitivity, 86%; specificity, 100%; positive predictive value, 100%; negative predictive value, 90%).
In no instance was a negative NAAT result used by clinicians as definitive evidence that a patient did not have extrapulmonary tuberculosis; in all but one case, patients were continued on antituberculous therapy until final culture results were available.
Multiple laboratory-based studies of the NAAT for M tuberculosis suggest the assay is both sensitive (73 to 100%) and specific (93 to 100%) in a wide array of extrapulmonary specimens.

Evaluating Diagnostic Tools for Sexually Transmitted Infections - RTI International
NAAT is used with urine specimens, which can easily be obtained in large-scale public health programs or population surveys.
Results of studies of the general population using these methods indicate that NAAT typically detects substantially more infected persons than traditional assays, that the majority of NAAT-detectable infections are asymptomatic, and that infected individuals do not exhibit the same high-risk behavioral profile of subjects identified in studies of clinic populations.
However, it is also possible that NAAT is identifying clinically inconsequential infections because of the assay's ability to detect low levels of viable organisms or amplifiable DNA from past infections that have not yet cleared.

 2002 Laboratory Guidelines - 3
Another approach is to use a NAAT to test specimens that yield results from an EIA or unamplified nucleic acid probe test that fall in a zone around the cutoff (i.e., gray zone).
By using a NAAT to retest specimens with gray-zone results from less sensitive and less expensive screening tests, the sensitivity and specificity of the screening test might be improved at less cost than switching entirely to the NAAT.
In particular, determining that NAAT specificity and sensitivity are not reduced is critical if a NAAT is performed on specimens collected, transported, and possibly processed by using non-NAAT procedures that might be less stringent with respect to preventing contamination and inhibition of amplification enzymes.

AEGiS-MMWR: Screening Tests To Detect Chlamydia trachomatis and Neisseria gonorrhoeae Infections --- 2002
When using a NAAT, any sacrifice in performance when urine is substituted for a traditional swab specimen is limited, thus reducing dependence on invasive procedures and expanding the venues where specimens can be obtained.
The increased sensitivity of NAATs is attributable to their ability to produce a positive signal from as little as a single copy of the target DNA or RNA.
Estimates of the differences in the performance of NAATs and non-NAATs are also uncertain because only a limited number of evaluations have been conducted that evaluated the two sets of tests by using the same set of study subjects (i.e., a head-to-head study).

A NAAT has high potential as an additional test after non-NAAT tests because of the increased sensitivity; however, this use of NAATs has received limited evaluation.

New HIV screening method, nucleic acid amplification testing (NAAT)
Adding a new HIV screening method, called nucleic acid amplification testing (NAAT), to standard HIV testing, researchers were able to uncover six percent more cases of HIV infection in urban STD and drug treatment clinics and HIV testing sites in Atlanta than with standard HIV antibody tests alone.
The research was presented at the 12th Conference on Retroviruses and Opportunistic Infections in Boston on February 25, 2005 by Frances Priddy, MD, assistant professor of medicine at Emory University School of Medicine.

How many test results run by different nucleic acid amplification tests (NAAT) and what combinations of specimens comprise the best infected-patient gold standard for evaluating the performance of a new NAAT for the detection of C.
Multiple different infected-patient gold standards for estimating swab and urine specimen sensitivity and specificity for one NAAT method were created by varying the number and combination of swab and urine comparator results obtained with two different NAATs.  Data from a large study of the performance of a new NAAT for the detection of C.
The effect on the performance characteristics of the NAAT under evaluation was determined when the available comparator NAAT results used to define the infected-patient standard were reduced from four tests to three tests to two tests.
