  Brainboost - Neostigmine
Neostigmine is an anticholinesterase agent which inhibits the hydrolysis of acetylcholine by competing with acetylcholine for attachment to acetylcholinesterase.
Neostigmine is metabolized in the liver by microsomal enzymes.
Neostigmine is given as an intravenous bolus of 2 to 2.5 mg over 1 to 5 minutes or as an intravenous infusion of 0.4 mg/h for 24 hours.

Neostigmine - Biocrawler   (Site not responding. Last check: 2007-11-05)
Neostigmine is a parasympathomimetic, specifically, a reversible cholinesterase inhibitor.
By interfering with the breakdown of acetylcholine, neostigmine indirectly stimulates both nicotinic and muscarinic receptors.
Neostigmine will cause slowing of the heart rate (bradycardia), for this reason it is usually given along with a parasympatholytic drug such as atropine or glycopyrrolate.

Neostigmine Bromide Tablets , SPC from the eMC
Neostigmine bromide has a slower onset of effect when given orally than when given parenterally, but the duration of action is longer and the intensity of action more uniform.
Neostigmine Bromide should not be used in conjunction with depolarising muscle relaxants such as suxamethonium as neuromuscular blockade may be potentiated and prolonged apnoea may result.
Neostigmine Bromide is a quaternary ammonium compound and is poorly absorbed from the gastro-intestinal tract.

Because neostigmine increases the effective concentration of acetylcholine, it causes such body changes as contraction of the pupils, increased activity of intestinal muscles, and increased secretion by the salivary and sweat glands.
Neostigmine and related drugs are also used to diagnose and control the neuromuscular disease myasthenia gravis.
Because neostigmine causes decreased fluid pressure in the eye it is used to treat certain types of glaucoma.

Neostigmine is a parasympathomimetic agent that has been recently investigated for use as an adjunct analgesic agent in the perioperative and peripartum period.
While the intrathecal administration of neostigmine produced useful analgesic effects in the postoperative period in some studies, the high incidence of adverse events, mainly nausea and vomiting, limit the clinical usefulness of this route of administration.
Intra-articular administration of neostigmine 500μg produced a useful analgesic effect in the postoperative period in several studies and was not associated with an increase in the incidence of adverse effects.

The technology is a combination of two drugs: neostigmine, used as a reversible anti-cholinesterase inhibitor, and glycopyrrolate, an anti-cholinergic and anti-spasmodic.
Neostigmine is known for its side effects on the gastrointestinal tract, including an increase in motility of the colon resulting in defecation, and on the cardiovascular system, leading to a slowing of the heart rate.
To control the unwanted side effects of neostigmine on the cardiovascular system without blocking the desired effects on gastrointestinal tract, the glycopyrrolate compound was added in an appropriate dose.

"Our study confirms earlier studies showing that neostigmine is an effective, safe and inexpensive way to decompress the colon in patients with acute colonic pseudo-obstruction," said Kimmey, who is professor of medicine/gastroenterology at UW School of Medicine and director of GI endoscopy at UW Medical Center.

Clinical Trial: The Early Reversibility of Rocuronium After Different Doses of Neostigmine
If rocuronium-induced paralysis could be chemically reversed within 10 to 15 minutes after the administration of an intubating dose, it may be an appropriate alternative in patients with contraindications to succinylcholine or in patients whom a difficult airway is anticipated.
Inhibition of the breakdown of acetylcholine allows the neurotransmitter to be present in the neuromuscular junction for a longer period of time, so that each molecule can bind repeatedly with the acetylcholine receptor.
The purpose of this study is to determine the dose of neostigmine necessary for the early reversal of rocuronium-induced paralysis.

Neostigmine is used to treat Myasthenia gravis, a disease which causes weakness of the muscles.
Neostigmine works by increasing nerve impulses to the muscles, reducing weakness and allowing movement.
Important: If your neostigmine dosage is too high you may start to experience some of the following symptoms; tummy cramps, anxiety, blurred vision, clumsiness or unsteadiness, diarrheoa, sweating, excessive salivation, panic sensation, increasing muscle weakness, muscle cramps or twitching, unusual irritability or nervousness, unusual tiredness or weakness or an urgent need to urinate.

Effects of neostigmine and glycopyrrolate on pulmonary resistance in spinal cord injury
Neostigmine, an acetylcholinesterase inhibitor, has been used successfully to rapidly decompress the colon in patients with acute colonic pseudo-obstruction [1].
We found that administering neostigmine combined with glycopyrrolate was followed by significant bronchodilation as measured by decreases in R5 (9%) and R20 (7%).
The combination of neostigmine with glycopyrrolate was associated with a significant decrease in resistances, demonstrating that cholingerically mediated bronchoconstriction is prevented by the addition of glycopyrrolate.

Effects of neostigmine and glycopyrrolate on pulmonary resistance in spinal cord injury
The objectives of the study were to determine the effects of intravenously administered neostigmine or neostigmine combined with glycopyrrolate on forced oscillation indices in persons with SCI.
Values were obtained before and after the intravenous administration of 2 mg of neostigmine alone and, on a separate day, before and after the administration of 2 mg of neostigmine combined with 0.4 mg of glycopyrrolate.
Neostigmine alone induced significant bronchoconstriction, whereas neostigmine combined with glycopyrrolate caused bronchodilation.
