Neutrophil granulocytes (commonly referred to as neutrophils) are a class of white blood cells and are part of the immune system.
Neutrophils are active phagocytes, capable of only one phagocytic event, expending all of their glucose reserves in an extremely vigorous respiratory burst.
In Familial Mediterranean fever (FMF), a mutation in the pyrin (or marenostrin) gene, which is expressed mainly in neutrophil granulocytes, leads to a constitutionally active acute phase response and causing attacks of fever, arthralgia, peritonitis and - eventually - amyloidosis.

Neutrophil granulocyte migrates from the blood vessel to the matrix, sensing proteolytic enzymes, in order to determine intercellular connections (to the improvement of its mobility) and envelop bacteria through phagocytosis.
Neutrophil granulocytes have an average volume of 330 femtoliters (fl) and a diameter of 12-15 micrometers (µm) in peripheral blood smears.

The author reviews several modern studies of granulocyte transfusion therapy and summarizes by stating that the bottom line is that it is not clear whether even large doses of granulocytes are useful in clearing infections or prolonging survival in neutropenic immunocompromized patients.
Although these results suggest that prophylactic granulocyte transfusions may be effective in limiting the period of neutropenia, it must be emphasized, however, that the published results to date represent preliminary findings and the efficacy of prophylactic granulocyte transfusion requires further study in large well-controlled trials.
Granulocytes should be transfused daily and at least a 4 day treatment is recommended with continuation after this time based upon clinical judgment and patient response.

In the marrow, the neutrophils exist in 2 divisions—the proliferative, or mitotic, compartment (myeloblasts, promyelocytes, myelocytes) and the maturation-storage compartment (metamyelocytes, bands, polys).
Some neutrophils do not circulate freely (marginal compartment) but are adherent to the vascular surface, and these constitute approximately half the total neutrophils in the blood compartment.
