 NCBoN Form for NC CRNA Recognition
In order to assure that CRNAs are meeting these regulatory requirements, each nurse anesthetist must verify his/her certification status with the NC Board of Nursing and thereafter on a periodic basis consistent with the nurse anesthetist’s certification renewal cycle.
Approximately 60 days prior to expiration of the national certification, the Board will send the CRNA a notice that the NC CRNA recognition update form must be completed and that their current national certification number and expiration date must be provided to the Board of Nursing.
Process for update of NC CRNA recognition: Once the graduate nurse anesthetist is verified to practice as a CRNA in North Carolina, they must update their NC CRNA recognition on a periodic basis consistent with the nurse anesthetist’s certification renewal cycle.

A certification received from a New Jersey massage, bodywork and somatic school does not entitle you to call yourself "certified" or use the abbreviation "CMT" until such time as you are certified by the New Jersey Board of Nursing.
If you hold a National Certification as a Massage, Bodywork and Somatic Therapist from either the National Certification Board for Therapeutic Massage and Bodywork (NCBTMB), or the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM) you must indicate the name of the certifying body so as not to be misleading to New Jersey consumers.
If you are licensed in another state as a Massage, Bodywork or Somatic Therapist you must identify the state of licensure so as to distinguish another state licensure from a New Jersey State Certification.
