Stroke can also be caused by the accumulation of a protein called amyloid within the artery walls, particularly in the elderly.
About 20% of strokes are hemorrhagic -- but the other 80% are caused by the opposite problem: too little blood reaching an area of the brain, which is usually due to a clot that has blocked a blood vessel.
This is called "ischemic stroke." This type of stroke can sometimes lead to a brain hemorrhage because the affected brain tissue softens and this can lead to breaking down of small blood vessels.

It seems that 4 stroke OBs may be a safer bet from the point of view that they do not self destruct if you have a lean running cylinder, and the more conventional pressurized lube system appears less prone to failure.
To me if you can deal with the service an outdrive requires, the 4 stroke car based engine is way simpler than a DI 2 stroke, or a 4 stroke OB which is much like a twin cam Japanese car engine.
