Body - Becoming a Woman - A guide to Polycystic Ovary Syndrome (PCOS): Oral Glucose Tolerance Test (OGTT)   (Site not responding. Last check: 2007-10-12)
An OGTT is a way to measure your body’s ability to use glucose.
If the OGTT shows that your blood glucose levels are higher than normal, your health care provider may tell you that you have “impaired glucose tolerance.” This often means that you are at risk for developing diabetes.
Your health care provider will look at OGTT results along with other test results to figure out the best treatment for you including exercise, weight loss (if you are overweight), and sometimes taking medication.

Demographic parameters (BMI, waist-to-hip ratio, age) and plasma glucose and insulin values from the OGTT were subjected to multiple linear regression to predict the metabolic clearance rate (MCR) of glucose, the insulin sensitivity index (ISI), and first-phase (1st PH) and second-phase (2nd PH) insulin release as measured with the respective clamps.
The goal of this study was to investigate the prevalence of diabetes mellitus and impaired glucose tolerance in the adult population of Catalonia and study their association with obesity, central obesity, hypertension and smoking habit.
After an OGTT, 90-min values of log plasma insulin and 90-min values of the ratio of log plasma insulin to blood glucose may be good indicators of insulin secretory properties in normal glucose-tolerant family members of type 2 diabetic subjects.

The OGTT requires a fast of eight to 12 hours, after which a person's blood glucose is measured before drinking a glucose-containing solution and then again two hours later.
The OGTT includes measures of blood glucose levels after a fast and after a glucose challenge.
On the other hand, the OGTT is more sensitive in identifying people with problems that may first appear only after high glucose intake.

 Glucose Tests: The Test
The OGTT may be ordered to help diagnose diabetes and as a follow-up test to an elevated blood glucose.
The American Diabetes Association recommends either the fasting glucose or the OGTT to diagnose diabetes but says that testing should be done twice, at different times, in order to confirm a diagnosis of diabetes.
Most pregnant women are screened for gestational diabetes, a temporary form of hyperglycemia, between their 24th and 28th week of pregnancy using a version of the OGTT, a 1-hour glucose challenge.

A total of 889 pregnant women who underwent at least one test of the two-step OGTT were studied retrospectively for adverse maternal and fetal outcomes.
Application of the latest ADA criteria to the two-step OGTT was determined to be appropriate for UAE pregnant women tested for GDM in the tertiary care setting.
Recently the ADA recommended lower cutoff values for both the 50g GLT, from 7.8 mmol/l to 7.2 mmol/l, and the 100g OGTT, from 5.8 (fasting), 10.5 (1 h), 9.2 (2 h) and 8.0 (3 h) mmol/l to 5.3, 10.0, 8.6 and 7.8 mmol/l respectively.

Effect on Glucose Metabolism by Adding Lo-Dose RTV to Reyataz: In Vitro study
Of note, based on glucose levels at 2 h on OGTT, one subject developed diabetes (2 h glucose 11.3 mmol/l), one became glucose intolerant (2 h glucose 7.9 mmol/l), and one nearly developed impaired glucose tolerance (2 h glucose 7.7 mmol/l).
The decreased insulin levels at 30 min during the OGTT are consistent with impaired first-phase insulin secretion.
Alternatively, the 10% decrease induced by lopinavir/ritonavir in glucose tolerance by AUC during the OGTT is small, and it is possible that a small change may have been missed on euglycemic hyperinsulinemic clamp or fasting glucose and insulin levels.

The use of the 2-h OGTT for diagnosis is given scant consideration on account of its poor reproducibility, which is substantially worse than that of fasting glucose or glycated hemoglobin (HbA1c).
The 2-h OGTT result identifies a substantial additional number of individuals at risk of microvascular diabetic complications and macrovascular disease who would be missed on fasting results.
The use of the OGTT to identify those with IGT for intervention aimed at the prevention of diabetes can be made more efficient by limiting it to high-risk individuals [12].

Soon after OGTT was introduced, it became apparent that use of the test led to overdiagnosis of diabetes mellitus (1).
The investigators concluded that because the prevalence of such isolated elevation increases with age, the use of FPG alone for diabetes screening or diagnosis may fail to identify most older adults at high risk for cardiovascular disease and should be reevaluated.
OGTT also may be considered in older adults, particularly women older than 65 years, with normal FPG, because 37.3% of normoglycemic individuals had abnormal glucose values on OGTT according to the fasting ADA criterion (31.4% had impaired glucose tolerance and 5.9% had diabetes mellitus) (6).

Glucose tolerance test - Wikipedia, the free encyclopedia
The test is usually used to test for diabetes, insulin resistance, and sometimes reactive hypoglycemia.
Usually the OGTT is scheduled to begin in the morning (0700-0800) as glucose tolerance exhibits a diurnal rhythm with a significant decrease in the afternoon.
A standard 2 hour OGTT is sufficient to diagnose or exclude all forms of diabetes mellitus at all but the earliest stages of development.

 OUTLINE
The oral glucose tolerance test (OGTT), once the gold standard for diagnosing diabetes mellitus, is now not recommended by the American Diabetes Association (ADA) for diagnosing either type 1 or type 2 diabetes, but continues to be recommended in a limited fashion by the World Health Organization (WHO) (1, 2).
There continues to be controversy over the use of the OGTT to classify individuals with impaired glucose tolerance and diabetes.
Furthermore, use of the OGTT and the WHO criteria to diagnose diabetes identifies approximately 2% more individuals as diabetic than if the ADA FPG criteria are used (7).

 Managing your Diabetes
People with IGT have blood glucose levels that are higher than normal but not high enough to say they have diabetes.
This condition is diagnosed using the oral glucose tolerance test (OGTT).
Though the fasting glucose test detects most diabetes cases, the OGTT is more sensitive in identifying people with blood glucose problems that may first appear only after a glucose challenge.

 Oral Glucose Tolerance Test
The oral glucose tolerance test (OGTT) measures the body's ability to use a type of sugar, called glucose, that is the body's main source of energy.
An OGTT is most commonly done to check for diabetes that occurs with pregnancy (gestational diabetes).
When done for this purpose, the test is called a glucose challenge screening test, and it is usually done during the 24th to the 28th week of pregnancy.

Defeat Diabetes: Postnatal Fasting Plasma Glucose Determines Need for Oral Glucose Tolerance Test   (Site not responding. Last check: 2007-10-12)
British researchers report, that in women with gestational diabetes, a postnatal oral glucose tolerance test (OGTT) may only be necessary in women whose postnatal fasting plasma glucose is 6.0 mmol/L or more.
But compared to the fasting plasma glucose test, the OGTT is time-consuming, unpleasant, and relatively expensive.
None of the women with a normal OGTT level had fasting glucose of 6.0 mmol/L or more, and the two tests were well correlated (r=0.62, P<0.0001).

Current Role of the Oral Glucose Tolerance Test
However, the OGTT has no role in the management of diabetes.

According to the American Diabetes Association (ADA) recommendations,[1] in the absence of ketoacidosis or nonketotic hyperglycemic coma, hyperglycemia must be documented on 2 separate occasions to diagnose diabetes.

It can be argued that if the fasting plasma glucose is found to be elevated, the OGTT should not be completed.

A reasonable approach for diagnosing diabetes in symptomatic subjects is to first obtain a random plasma glucose.

However, in comparison with the 3-hour, 100-g test, there are fewer data on the performance of the 2-hour, 75-g test in pregnancy.

In summary, the OGTT can play an important role in both the diagnosis of diabetes and the screening for diabetes and prediabetes.

Schizophrenia Daily News Blog: Diabetes Screening Process Validated for Patients With Schizophrenia
However, other data has been emerging showing that the FPG test is sometimes insufficient for detecting glucose metabolism abnormalities in this patient population, and therefore some doctors are suggesting that the oral glucose tolerance test (OGTT), considered to be the "gold standard" test for diabetes and glucose metabolism problems, must be used in some cases.
On the other hand, the OGTT requires the patient to be in the lab for several hours and requires multiple blood draws.
The FPG is therefore faster and less expensive than the OGTT, which is a factor in not simply screening all patients with the most effective test for detection of glucose metabolism abnormalities - the OGTT.

In addition, although FPIR, a good marker of b-cell integrity, was higher in subjects with NGT than in subjects with IGT or diabetes (Fig.

Acromegaly Diagnosis   (Site not responding. Last check: 2007-10-12)
Without an OGTT, an average level of <2.5 ng/mL from multiple random samples is considered normal.
However, since elevation in GH is a basic abnormality in acromegaly, basal GH levels may be examined during the diagnostic phase, as well as during the ongoing monitoring of acromegalic patients.
When acromegaly is suspected, an oral glucose tolerance test (OGTT) is used to evaluate the response of GH to "glucose challenge." In this test, 75 or 100 g of glucose are administered in an oral solution to a patient who has fasted overnight.
