 | | Accidents are reported to OMV on the SR-10 personal accident report form which is supposed to be completely filled out and turned in to this office by all owner/drivers involved in an accident with vehicle damage of $500 or more or any accident in which there was any physical injury sustained by anyone involved. |
 | | By state law, it is the responsibility of the vehicle owner to file the SR-10 with OMV and the owner must complete and sign the document before filing it with this office along with proof that the vehicle was insured at the time of the accident. |
 | | Many people believe that OMV reports to their insurance company that they were at fault in an accident when, in truth, an official driving record only shows that you were involved in the accident as the owner, driver, or both. |
