Olecranon Bursitis - DynoMed.com   (Site not responding. Last check: 2007-11-02)
The swelling associated with olecranon bursitis may develop gradually, representing a chronic condition.  It may also occur suddenly as the result of infection or traumatic injury.  Pain is usually more intense and the motion of the elbow more limited when bursitis occurs because of an injury or infection.
The onset of olecranon bursitis may be the result of an injury to the elbow joint from a sport such as baseball, tennis, racquetball, or running.
If a significant period of time has passed since olecranon bursitis has occurred in the elbow, it is quite possible that the surrounding tendons and muscles may have contracted and lost their elasticity due to the severity or acuteness of the original injury and subsequent infection.

Ulna - Wikipedia, the free encyclopedia
It is bent forward at the summit so as to present a prominent lip which is received into the olecranon fossa of the humerus in extension of the forearm.
The compact layer is continued onto the back of the olecranon as a plate of close spongy bone with lamellæ parallel.
Below the coronoid process there is a small area of compact bone from which trabeculæ curve upward to end obliquely to the surface of the semilunar notch which is coated with a thin layer of compact bone.

eMedicine - Olecranon Fractures : Article Excerpt by: James W Pritchett, MD, FACS
Olecranon fractures are a diverse group of injuries, ranging from simple nondisplaced fractures to complex fracture dislocations of the elbow joint.
The olecranon process rarely is broken in children because, in early life, it is short, thick, and relatively much stronger than the lower end of the humerus.
Although olecranon fractures generally are isolated injuries, a high index of suspicion for associated injuries is warranted in the evaluation of patients with multiple trauma.

MANAGEMENT OF COMMINUTED FRACTURES OF THE OLECRANON BY TENSION BAND WIRING
Since it is intra-articular and the olecranon effectively functions as the fulcrum of the lever arm of the elbow, it is necessary to restore precise anatomical alignment with rigid fixation so that early movement can be encouraged.
The goal was to replace the comminuted section of the mid portion of the olecranon with a bone block from the iliac crest while preserving the anatomical curvature of the notch of the trochlea.
After radiological assessment of the length of the ulna and the contour of the trochlear notch, two 0.8 mm wires were anchored to the bone block and to the shaft of the ulna separately, passed around the Kirschner wires and twisted in a figure-of-eight.

Olecranon osteotomy for exposure of fractures and nonunions of the distal humerus.
Twelve patients (27%) had removal of the wires used to repair the olecranon: in 6 patients, this was for symptoms related to the wires (13%); 1 for septic olecranon bursitis, and 5 at the time of another procedure (elbow capsular release in 4 patients and submuscular ulnar nerve transposition in 1).
Olecranon osteotomy can be used for exposure of the distal humerus with a low rate of complications when specific techniques are used.

The Physician and Sportsmedicine: Olecranon and Prepatellar Bursitis
The cause is typically trauma, either due to repetitive extremity movement or to acute trauma to the olecranon or patella.
Olecranon and prepatellar bursitis in active patients has three common presentations, and treatment is dictated by the type, the patient's activity or sport, and the desired timing of return to activity.

 Paediatric Olecranon fracture
Olecranon fractures are uncommon in children, accounting for between 4% and 7% of all childhood elbow fractures.
The ossification centre may be bipartite and eccentric, and as the proximal ulna grows, the growth-plate orientation alters from transverse to oblique.
It is difficult to predict in the valgus injury (metaphyseal olecranon and radial neck fracture) when an associated radial neck fracture will cause an increase in carrying angle, ideally angular deformity at the radial neck should be reduced.

The Physician and Sportsmedicine: Olecranon Fracture Sustained in Arm Wrestling
This case of an arm wrestler who sustained an olecranon fracture illustrates the potential effect of violent muscle contraction in arm wrestling and emphasizes the importance of radiographic evaluation.
Because of its immediately subcutaneous position, the olecranon is especially vulnerable to direct trauma, which is the mechanism of injury for most olecranon fractures (5).
Olecranon fractures are also not uncommon secondary to indirect loading of the joint, such as happens with a fall on the upper extremity.

Elbow Bursitis   (Site not responding. Last check: 2007-11-02)
Elbow bursitis or olecranon bursitis is an inflammation of a bursa, the lubricating fluid that permits smooth gliding action between bones, tendons or muscles.
The term olecranon bursitis is a combination of the word “olecranon,” which is the bony prominence of the elbow, and “bursitis,” the inflammation of a bursa.
Olecranon bursitis is typically the result of joints being torn by excessive force.

Excision of the fracture fragment and reattachment of the triceps tendon may be indicated in a select group of elderly patients with osteoporotic bone in whom the olecranon fracture involves less than 50% of the joint surface or when the fragment is too small or comminuted for successful internal fixation.

Rather than using an olecranon osteotomy to expose the articular surface of the distal humerus, the remaining triceps tendon was dissected from the tip of the olecranon.
Osteotomy of the olecranon was avoided initially since adequate exposure of the articular surface of the distal humerus was thought to be achievable by the detachment of the remainder of the triceps tendon alone.
