Oligospermia is defined as less than 20 million sperm per mL, severe oligospermia is less than 5 million sperm per mL, and azoospermia is defined as no sperm present.

Possibilities of association of azoospermia and oligospermia with some HLA system antigens.Modern knowledge of the HLA system structure and its role in the organism supports the statement that the presence of HLA antigens in a person increases his/her predisposition to some diseases.
This study was an attempt to obtain insight into the possibilities of the association of HLA antigens with azoospermia and oligospermia of an unknown aetiology.
The condition of disordered spermiogenesis was confirmed on the basis of anamnestic data, objective genital examination and spermiogram, by which secondary causes were excluded and unknown aetiology was confirmed.

A markedly elevated serum FSH usually is associated with azoospermia or severe oligospermia and usually indicates a difficult-to-treat germ cell defect (primary testicular failure).
Because hyperprolactinemia has also been reported to cause oligospermia, serum prolactin should be measured when a patient has a low serum testosterone level without an associated increase in LH, as well as symptoms of decreased libido, decreased ejaculate volume, and especially with galactorrhea.
Men previously considered untreatable with conditions such as congenital bilateral absence of the vas deferens and severe untreatable oligospermia are now potentially able to initiate a pregnancy with ICSI.

 Management of Male Infertility
In evaluating patients with azoospermia or severe oligospermia, obstruction at the level of the ejaculatory ducts must be ruled out.
Although it is difficult to predict which patients will respond to clomiphene, patients not likely to benefit from this therapy include those with elevated baseline serum FSH levels, severe oligospermia, asthenospermia, teratozoospermia or azoospermia, and those with extremely abnormal testis biopsy results.
Another area of interest in the study of idiopathic male subfertility is the potential effect of ROS on sperm function and fertility.
