Orbicularis Oris Muscle   (Site not responding. Last check: 2007-10-08)
The orbicularis oris is a sphincter muscle that encircles the mouth.
It lies between the skin and the mucous membranes of the lips, extending upward to the nose and down to the region between the lower lip and chin.
It is sometimes called the kissing muscle because it causes the lips to close and pucker.

The pre-orbital portion of the Orbicularis oculi muscle arises from insertions on the frontal process of the maxillary bone in front of the anterior lacrimal crest and from the common medial canthal tendon.
In contrast, the palpebral portion of the Orbicularis oculi muscle consists of two hemiellipses of muscle that are fixed medially and laterally at the medial and lateral canthal tendon complexes.
The Orbicularis oris is not a simple sphincter muscle like the Orbicularis oculi; it consists of numerous layers of muscle fibers surrounding the opening of the mouth that are oriented in many different directions.

Depressor labii inferioris, lateral to the mentalis, attaches inferiorly to the mandible and merges superiorly with its contralateral partner and the orbicularis oris and draws the lip inferioly and slightly laterally.
Orbicularis oris originates from the skin and fascia of the lips and inserts in the skin and fascia of the lips.

It arises from the oblique line of the mandible, between the symphysis and the mental foramen, and passes upward and medialward, to be inserted into the integument of the lower lip, its fibers blending with the Orbicularis oris, and with those of its fellow of the opposite side.
The Triangularis (Depressor anguli oris) arises from the oblique line of the mandible, whence its fibers converge, to be inserted, by a narrow fasciculus, into the angle of the mouth.
At its origin it is continuous with the Platysma, and at its insertion with the Orbicularis oris and Risorius; some of its fibers are directly continuous with those of the Caninus, and others are occasionally found crossing from the muscle of one side to that of the other; these latter fibers constitute the Transversus menti.

Orbicularis oris muscle - Wikipedia, the free encyclopedia
The Orbicularis oris is not a simple sphincter muscle like the Orbicularis oculi; it consists of numerous strata of muscular fibers surrounding the orifice of the mouth but having different direction.
Of the former, a considerable number are derived from the Buccinator and form the deeper stratum of the Orbicularis.
Some of the Buccinator fibers—namely, those near the middle of the muscle—decussate at the angle of the mouth, those arising from the maxilla passing to the lower lip, and those from the mandible to the upper lip.

The orbicularis oris is innervated by buccal branches of VII; the depressor anguli oris, depressor labii inferioris, and mentalis are innervated by the marginal mandibular branch of VII; and the lip elevators are innervated by buccal and zygomatic branches of facial nerve VII.
Innervated orbicularis oris flaps are mobilized by an incision along the lateral border of the flap that overlies the outer border of the orbicularis oris muscle (see Image 2A, Image 3A).
The mental nerve is identified as it exits from the mental foramen and its branches to the flap are dissected free from the surrounding loose areolar tissue to permit transfer.

Orbicularis oris is the sphincter muscle around the mouth, forming much of the tissue of the lips.
As an expressive muscle, four relatively distinct movements can be produced by orbicularis oris, a pressing together, a tightening and thinning, a rolling inwards between the teeth, and a thrusting outwards.
Orbicularis oris is innervated by the lower zygomatic, buccal, and mandibular branches of the facial nerve (VII) and is supplied with blood by the facial artery.

Presumably a muscle rupture occurred, and the fibrous or scar tissue interposition produced a lengthening of the circumference of the orbicularis oris.

In this procedure, the mucosa is excised from the mucocutaneous junction posteriorly as far as the lesion extends, and to the depth of the orbicularis oris musculature.
The advantage of this technique is that the continuity of the orbicularis oris is largely maintained and it can also be used bilaterally or with other methods to close even larger defects.
