On May 10, 2006, at approximately 9:00 a.m., a 2001 Toyota Camry driven by ROBERT L. TOWNES, age 47, from Richland, Washington, was southbound on Highway 99W near milepost 87 when it crossed the centerline and collided head-on with a northbound 1986 International truck towing a single semi-trailer.
Oregon State Police troopers from the Albany Area Command office are conducting the investigation.

A Philomath woman died Tuesday night when the car she was driving struck the back of farm equipment being moved on Highway 99W about two miles north of Monmouth.
On July 26, 2005 at approximately 9:37 p.m., a 2001 Volkswagon Beetle driven by KAREN A. STUEVE, age 54, from Philomath, was southbound on Highway 99W at Clow Corner Road when it struck the back of a slow-moving Swather operated by LUKE D. LAFAYETTE, age 15, from Monmouth.
A Swather is an implement of husbandry and, according to investigators, was properly equipped for travel on the highway.

Orsborn was arrested and lodged in the Polk County Jail for criminally negligent homicide, driving under the influence of intoxicants, reckless driving, recklessly endangering another person, and assault in the fourth degree.

The bridge on Oregon Highway 99W just south of downtown is made up of two separate steel structures carrying northbound and southbound traffic.
