Topic: Oxacillin

Oxacillin - Drugs & Vitamins - Drug Library - DrugDigest
Oxacillin kills certain bacteria that cause infection, or stops their growth.
Take oxacillin 1 to 2 hours before or at least 2 hours after eating; taking it with food can make it less effective.
If you are diabetic and taking large doses of oxacillin, you may get a false-positive result for sugar in your urine.

Oxacillin is used to treat many different types of bacterial infection, especially those caused by staphylococcus bacteria ("staph" infections).
Oxacillin is in the FDA pregnancy category B. This means that it is unlikely to harm an unborn baby.
Oxacillin should be taken at evenly spaced intervals throughout the day and night to keep the level in your blood high enough to treat the infection.

 MRSA
This phenomenon is termed heteroresistance and occurs in staphylococci resistant to penicillinase-stable penicillins, such as oxacillin.
Oxacillin screen plates can be used in addition to routine susceptibility test methods or as a back-up method.
Oxacillin is more resistant to degradation in storage and is more likely to detect most heteroresistant strains.

The antibiotic oxacillin is also strongly bound to albumin and is often administered to patients receiving phenytoin.
Oxacillin at a concentration of 15 µg/ml caused no significant displacement of phenytoin in a serum pool prepared from patients receiving phenytoin.
In one hypoalbuminemic patient receiving both phenytoin and intravenous high-dose oxacillin, the free phenytoin fraction was 22.5% before oxacillin therapy, 24.1% 12 hours after first dose of oxacillin, and 27.2% after 60 hours, indicating the possibility of in vivo displacement of phenytoin by oxacillin.

BioMed Central | Full text | Cloxacillin versus vancomycin for presumed late-onset sepsis in the Neonatal Intensive ...
We hypothesized that restricting vancomycin use to oxacillin resistant organisms only, for late onset sepsis in the NICU would not be inferior to a non-restricted policy.
In 27 of these cases, antibiotics were switched to vancomycin, however 14 (52%) of these 27 infants had clinically improved from their sepsis before the start of vancomycin: 10 had returned to their baseline clinical status, 2 had a negative blood culture and 2 had both before initiation of vancomycin.
Reserving vancomycin for the treatment of sepsis caused by organisms confirmed to be oxacillin resistant is an important measure to control the spread of vancomycin resistant organisms.

MRSA is of special concern because it is resistant not only to methicillin, oxacillin, and nafcillin but also to all other ß-lactams, including cephalosporins, imipenem and meropenem, and aztreonam.
Recently, strains resistant to methicillin and oxacillin but susceptible to many non-b-lactam agents such as clindamycin and gentamicin have been recovered from a few patients who appear to have acquired the organism outside of healthcare settings.
For this reason, methicillin or oxacillin resistance is not detected well by some susceptibility testing systems (Table 2).

BD - Diagnostic Systems: Oxacillin Screen Agar (Mueller Hinton Agar with 6 µg/mL Oxacillin & 4% NaCl)
Oxacillin Screen Agar (originally named MRSA Screen Agar) was originally developed for the detection of methicillin-resistant Staphylococcus aureus (MRSA).
These strains are resistant to penicillinase-resistant penicillins (PRPs), such as methicillin, oxacillin and nafcillin.
Since the method to detect MRSA uses the same inoculum as the Bauer-Kirby antimicrobial disc susceptibility test procedure, the oxacillin screen test may be conveniently performed on isolates at the same time as routine susceptibility testing.

Several mechanisms can lead to the development of oxacillin resistance and several classes of MRSA have been identified based on these mechanisms (Table II).
In a more recent publication, all classes of oxacillin resistance were detected 100% of the time when the zone around a cefoxitin or moxalactam disk was <27 mm or <24 mm respectively.
