The Padma Vibhushan was originally established, in January 1954, as the "Pahela Varg" or "First Class" of a three-class "Padma Vibhushan" award.
In 1957, the badge itself was altered to be of burnished bronze, with all embossing in white gold.

NEW DELHI: President A.P.J. Abdul Kalam conferred the Padma Vibhushan, the Padma Bhushan and the Padma Shri on 53 persons in the second part of the investiture ceremony at the Rashtrapati Bhavan on Wednesday.
The Padma Bhushan was conferred on Devaki Jain for her contribution to social justice and women empowerment.
The Padma Bhushan was also conferred on Hira Lall Sibal for his contribution to the cause of justice, Hindi writer Kamleshwar and well-known authority on the Constitution M.V. Pylee.

I do not mean to say that all the persons that are not worthy of national honor for their respective contributions in enlisted fields have been given the honor, but lack of any codified and legalistic criterion to ensure that no worthier is left behind has made the entire exercise questionable.
Selection of persons for Padma title, though the criteria are not yet codified, is touted to be based on how far useful to society are the life long contributions of the person under consideration.
But she was first conferred with Padma Bhushan in 1954, then with Padma Vibhushan in 1975 and finally with Bharat Ratna in 1998.
