When you plug a patch cord into the top (output) jack on an HN patch bay, the output signal wired to that top jack stays connected to the (input) jack below it and therefore continues to be connected to the equipment wired to that input jack.
Patch cords connected to the top jack have access to the output signal of the device wired to that jack.
Patch cords connected to the bottom jack have access to the input of the device wired to that jack.

I work with another engineer who is of the philosophy that when physical patches are used, they should not remain in the bay for any longer than they are being used.
Patch bays bring all jacks in the studio to one place and provide easier ways of complex signle routing.
Of course, I can see the benefit in occasional cycling of patches and cleaning of connections, but I believe that to clear patch bays after every single show is far more destructive than leaving a cord in indefinitely.

When you patch into the front top, you essentially split the signal coming in (the top rear) but signal continues to connect thru the bottom front to bottom ear.
However, when you patch into the bottom front, you interrupt the signal from the top front to bottom front, or "break the normal" and the bottom rear now sees, the signal you patched into the bottom front.
Patching unbalanced into a balanced bay is ok as the ring and shield short in the bay, however doign the opposite floats the ring or neutral, which can cause noise problems and level loss.

The DACS MIDI Patch Bay ($275), one of its oldest products, was recently re-engineered and is now available in the US through Independent Audio.

The patch bays were bought used and although the snake was already soldered to it, a lot of the soldering was shitty and had to be re-done.
It was a lot of work, and this is it's second incarnation: I failed miserably at my first attempt to wire this up last year, but since have been able to educate myself on the electrical principles of how patch bays actually work and on soldering technique as well.
There are some bays where the grounds are strapped already from the manufacturer.

Every single Pro Sudio in the world uses patch bays and there is no extra noise, or humm or buzz if they are built correctly.
FYI...The $1000+ bay isn't just for the bay, that would a full wired 96 point TT bay, with a average length of wire coming off of it.
Very old bays, it doesn't matter if it cost's 10 bucks or $2000 because the metal over a long period of time builds up corrosion, which will lead to intermitant connections.
