  PA-01-124: PATIENT-CENTERED CARE: CUSTOMIZING CARE TO MEET PATIENTS’ NEEDS
For the purposes of this PA, patient-centered care is defined as health care that establishes a partnership among practitioners, patients and their families (when appropriate) to ensure that decisions respect patients’ wants, needs and preferences and solicit patients’ input on the education and support they need to make decisions and participate in their own care.
In addition, the content of care is primarily defined as the reimbursable exchange that occurs within the context of a clinical encounter (outpatient visit, hospitalization) in a specified setting rather than the response of clinicians to patients’ needs that occurs over time and across settings.
However, there is an emerging consensus that the role of patients (and families, as appropriate) as co-managers of their health and health care has been under-estimated and under valued as a critical component of improvement efforts.
