Patriarchal blessing - Wikipedia, the free encyclopedia
In Roman Catholicism and Eastern Orthodoxy, a patriarchal blessing is a blessing given by a Patriarch.
In the Church of Jesus Christ of Latter-day Saints, a patriarchal blessing is a unique blessing pronounced upon a church member by a church leader ordained to the Priesthood office of Patriarch (sometimes referred to as Evangelist).
The purpose of a patriarchal blessing is (1) to identify the tribe of Israel to which a Latter-day Saint belongs, (2) to bless the member with special gifts, (3) to give advice or help to the individual (often this includes foretelling of possible future events, opportunities, and temptations).

 The Power of Patriarchal Blessings
I want to refer to three other patriarchal blessings besides your own that I believe were given for you and should be as clearly written in your aspirations as any specific promise or counsel you have from your own personal blessing.
And the blessing I have in mind that reflects your past, which all patriarchal blessings sooner or later do, is section 86 of the Doctrine and Covenants.
And the blessings I have quoted, and I suspect your own in most instances, refer to the need-absolute need-get the power of the Lord Jesus Christ into the very veins of your blood and transform you and purge you of whatever is amiss or whatever poisons you have taken.

The patriarchal blessing may not be necessary for salvation, but it is a guidepost; a white line down the middle of the road; a series of stakes around the mountain pass with reflector buttons in them so that whenever needed in the darkness and in the storm, they are available.
In each blessing, the patriarch will declare, under inspiration, the literal blood lineage of the person to be blessed and then, as moved upon by the Spirit, make a statement as to possibilities and the special spiritual gifts, cautions, instructions, admonitions, and warnings as the patriarch may be prompted to give.
Patriarchal blessings are revelations to the recipients -- a white line down the middle of the road to protect, inspire, motivate toward activity and righteousness.

Patriarchal Priesthood - Wikipedia, the free encyclopedia

After the death of Hyrum Smith in 1843, his position as Presiding Patriarch was passed among Hyrum's descendents until 1979, when the Church placed the current Patriarch, Eldred G. Smith, on Emeritus Status, although he was not released from his position.
Since the office of Patriarch is a Priesthood office to which one is ordained and a General Authority position to which one is called, Eldred G. Smith's ordination remains valid even if the position is not currently filled.

Patriarchal Blessings -- past and present
Blessings are given, as needed, to those who are sick or have other troubles in their lives.
The process of receiving such a blessing is actually rather simple: after an interview with the bishop to ensure the worthiness of the blessing candidate, an appointment is made with the stake patriarch[2].
Though many find their blessings to be a source of inspiration, in my case it was one of a few critical junctures which eventually led to my leaving the Church.

An essential component of each patriarchal blessing is a statement of lineage that generally connects us to God by designating for us a place in the family of Israel, commonly of the tribe of Ephraim or Manassah.
These patriarchal blessings are a way of knowing in a deeply personal and profound manner that regardless of our circumstance, fortune, or status, we are endowed with the great love of God.

 LDS Patriarchal Blessings
Unlike the Old Testament blessings given by a father on his deathbed to his sons, the LDS blessings are given by non-relatives to various members of the church as a sort of road map for their lives and declares their lineage through one of the tribes of Israel.
Thus the blessings "of Abraham, Isaac and Jacob" are conferred.
In conclusion, we see that the father’s blessing on his children in the Old Testament was far different than the LDS concept of an official office of Patriarch to declare ones lineage and future.
