PeerReview would alleviate this risk, provided the community has a suitable number of technically trained people reading over the changes.
A problem with any PeerReview system is that it makes it impossible to do anything quickly; one would basically have to debug a proposed change oneself because of the FileReplacement lag.
PeerReview is not sufficient protection for a CommunityProgrammableWiki, because any sufficiently reflective SPW would be able to have the very peer review mechanisms themselves edited -- and potentially removed or subverted.

I think you bring up an interesting point: PeerReview can limit the opportunity for a ChangeFailure, by nipping a problem in the butt early on.
PeerReview limits ChangeFailure, by applying the community's wisdom to a problem.
PeerReview also acts as a ColdBlanket if applied too early, so it also slows change.

PeerReview consists of two parts, the PeerPart and the ReviewPart.
This is in my opinion given short shrift here, and is a large part of what I've been trying to address with systems such as the ScoopEngine.
One technical option to ensure the community is being diligent with PeerReview is to ensure each edited page was matched with at least one non-author view.

A special case of PeerReview is giving EditHints as done by a FriendlyPeerContributor.
