From an ISP point of view peering is beneficial as it reduces the amount of traffic passed on to your transit provider (hence lowering the cost of that link).
Traditionally peering happens between large players in the Internet market such as Telcos and ISPs, however there are also benefits for large sources or sinks of traffic to peer at a peering point for the same reasons as an ISP (it reduces their reliance on purchased transit).
Peering breaks down when peers are not honest brokers for their customers--widespread peering increases redundancy and reduces the pressure on long-distance links, but there are certain pricing schemes which do not provide incentive for peers to do this.

In a few cases we were able to set up peering by running a crossover cable between our router and a peer's router, but never having managed to get more than one peer in a given facility, running the peering through a switch made very little sense.
I didn't go after peering with any nationwide providers, as my employer was probably too small for them to notice, so I don't know if there would have been any interest in a Detroit peering point from larger networks.
If the goal is to make peering easy enough that local ISPs will actually want to do so, it makes a lot more sense to put the exchange point in a colo facility where many of the ISPs already are.

XchangePoint promises a 10-day lead time on connection to its peering point which uses a dark fiber loop from Level 3, routers from Extreme Networks and optical multiplexing equipment from Munich-based ADVA Optical Networking.
Although XchangePoint does charge a quarterly rental fee, it isn't reliant on it, as peering clubs are on their membership fees, to keep their networks ahead of the technology curve, especially investment in WDM optical multiplexing equipment.
