  Lower leg
The peroneus longus is a superficial muscle that runs along the upper part of the outside of the leg.
It originates from the upper shaft of the fibula (lower leg bone), descends across the outer side of the calcaneum (heel bone), crosses the sole of the foot and is inserted in the outer side of the base of the metatarsal bone of the big toe.
The peroneus tertius arises from the lower third of the fibula (lower leg bone) and the adjacent interosseous membrane and continues downward and slightly forward, across the ankle, where it tapers into a narrow, flat tendon that passes over the extensor digitorum brevis and inserts into the fifth metatarsal bone (little toe).

 ANATOMY
Peroneus brevis and peroneus longus, located on the lateral side of the leg, are the main evertors of the foot; in addition, they assist with plantar flexion of the ankle.
Peroneus longus crosses the cuboid tuberosity, runs in a groove on the underside of the cuboid bone, and continues across the sole of the foot to insert on the first metatarsal and medial cuneiform.
Flexor digitorum longus, located deep inside the calf, is primarily a plantar flexor of the second through fifth toes; however, like flexor hallucis longus, it also assists with plantar flexion of the ankle and inversion of the foot.

The peroneus longus courses distally to lie inferior to the peroneal tubercle, passes through the cuboid groove, and inserts on the base of the first metatarsal and medial cuneiform.
Tendinosis of the peroneus brevis and longus is not uncommon.
Diastasis of the os peroneum with superior migration of the proximal fragment is highly suspicious of rupture or significant attrition of the peroneus longus.

Peroneus Brevis Is a More Effective Evertor Than Peroneus Longus - American Orthopaedic Foot & Ankle Society
The purpose of this study was to test the hypothesis that peroneus brevis is a more effective evertor than peroneus longus.
Conclusions/Significance: Loading of peroneus brevis tendon in heel rise was found to result in more eversion of the talonavicular and subtalar joints than loading of peroneus longus tendon.
At the early heel rise position, 259 N load on the peroneus brevis produced an average 1.6 ± 3.5¡ of talonavicular eversion, while the same load on the peroneus longus produced an average 0.5 ± 4.5¡ of inversion; this difference was significant (p = 0.009).

The peroneus longus muscle originates from the lateral condyle of the tibia and the head of the fibula.
The tendon of peroneus longus courses behind the peroneus brevis tendon at the level of the ankle joint, travels inferior to the peroneal tubercle, and turns sharply in a medial direction at the cuboid bone.
Its tendon courses anterior to the peroneus longus tendon at the ankle.

The Peroneus Longus tendon, like the tibialis tendon, provides the surgeon an excellent graft for primary, as well as revision ACL and PCL reconstructions.
The use of this graft allows the physician to create a single-looped stitched configuration for use with interference screw fixation or as a single length of tendon looped over a TransFix™ pin for femoral fixation.
Peroneus Longus tendons may also be used for small joint or podiatric surgical applications such as lateral/medial ankle ligament reconstruction or hand / wrist / elbow repair.

Persistent Pain After Ankle Sprain: Is A Peroneal Tendon Injury The Cause?
The peroneus brevis tendon is pushed between the peroneus longus and the fibula, and the inversion may snap the tendon against the side of the fibula.
In cases of a split tear, one will see the peroneus longus tendon sitting between the split pieces of the brevis tendon and/or the brevis tendon may be partially dislocated lateral to the fibular edge with one split medial and one split lateral to the lateral fibular border.
The longus tendon will often tear lateral to the cuboid or plantar to the cuboid region, and therefore is a more difficult area to treat and repair.

It arises from the lower two-thirds of the lateral surface of the body of the fibula; medial to the Peronæus longus; and from the intermuscular septa separating it from the adjacent muscles on the front and back of the leg.
The fibers pass vertically downward, and end in a tendon which runs behind the lateral malleolus along with but in front of that of the preceding muscle, the two tendons being enclosed in the same compartment, and lubricated by a common mucous sheath.
It then runs forward on the lateral side of the calcaneus, above the trochlear process and the tendon of the Peronæus longus, and is inserted into the tuberosity at the base of the fifth metatarsal bone, on its lateral side.
