Phalloplasty - Wikipedia, the free encyclopedia
Phalloplasty refers to the (re-)construction of a penis or, sometimes, to artificial modification of the penis by surgery, often for cosmetic purposes.
The basic procedures are the same, although surgery on cisgendered men is usually a lot simpler, since the urethra still ends in the front of the genital area, whereas the urethra of transmen ends near the vaginal opening and has to be lengthened considerably.
With all types of phalloplasty in transmen, the labia (see vulva) are united to form a scrotum, where prosthetic testicles can be inserted.

Metoidioplasty - Wikipedia, the free encyclopedia
Metoidioplasty is a variant of phalloplasty for transmen.
During this procedure the clitoris is released from its "hood", and the urethra is lengthened with the labia minor to end at its tip.
This procedure is much simpler than full-scale phalloplasty, and usually has much less complication.

Isolated amputation of the noncarcinogenic penile glans is an extremely rare event, Immediate replantation as a composite graft or by microvascular techniques is the treatment of choice, Secondary reconstruction of the penile glans may be indicated after necrosis as a sequela of infection, surgical resection for malignancy, traumatic loss, and self-mutilation.

Depending on the type of phalloplasty procedure, skin is typically taken from the abdomen, groin/leg, and/or forearm and grafted into the pubic area.
This is due to the nature of the skin of the forearm (areas of the forearm are of good consistency and often are fairly hairless) as well as the nerves and blood vessels that are able to be harvested with that skin.
Phalloplasty procedures also tend to be very expensive (between $50,000 to $150,000) and are not typically covered by insurance.

A population of 23 transsexuals accepted for phalloplasty was compared to a population of 40 who had undergone such surgery between six and one hundred and sixty months previously.
He noted that only 39% of female transsexuals were satisfied with the results of their phalloplasty, as compared with the 91% of male patients who were satisfied with a vaginoplasty.
He quotes also Lothstein, who spoke at the same conference, and found that whilst all the female transsexuals undergoing phalloplasty were satisfied with the surgical results and two-thirds of patients reported improved sex lives, half "entertained suicidal ideation" and two "wished their penises were larger and worried whether they could ever satisfy a woman".

Phalloplasty involves constructing a penis from the inner forearm skin (nondominant side) and vaginal tissue and attaching it to the vaginal area.
The forearm skin is grafted along with its nerves, arteries, and veins and formed around a plastic catheter tube, which will serve as the urethra and allow for urination once connected to the female urethra.
Hardening of the urinary tract and tissue death in the new penis are complications of phalloplasty.

The current recommendations are that with such counselling, most FTM's develop coping mechanisms to deal with their lack of a penis, and their partners whether female or male learn to respect the emotional limitations that FTM's face.
These are notorious for their failure rate, and one of the main problems currently incurred by FTM's who undergo phalloplasty is catastrophic failure of the site where such a tube is connected to the former urethral channel.
As such, it is positively cruel and inhumane to demand that FTM's should undergo phalloplasty which will leave most of them incontinent, unable to work, in great pain, severely scarred and socially isolated.
