Topic: Pharyngeal pouch

  Chronic cough
A pharyngeal pouch or Zenker's diverticulum is a pulsion diverticulum that arises between the cricopharyngeus and thyropharyngeus muscles of the inferior pharyngeal constrictor.
As pharyngeal pouches usually arise in the elderly, their management is often determined by the general fitness of the patient and the presence of any co-morbid conditions.
Surgery for a pharyngeal pouch, particularly if an external approach is used, is often associated with a significant risk of complications and peri-operative mortality rates of 2% have been reported.

The first pharyngeal pouch is prolonged dorsally to form the auditory tube and the tympanic cavity; the closing membrane between the mandibular and hyoid arches is invaded by mesoderm, and forms the tympanic membrane.
From the third pharyngeal pouch the thymus arises as an entodermal diverticulum on either side, and from the fourth pouches small diverticula project and become incorporated with the thymus, but in man these diverticula probably never form true thymus tissue.
From the fifth pouches the ultimobranchial bodies originate and are enveloped by the lateral prolongations of the median thyroid rudiment; they do not, however, form true thyroid tissue, nor are any traces of them found in the human adult.

A pharyngeal pouch (also known as a Zenker's diverticulum) is an out pouching of the pharyngeal mucosa through the muscle wall of the organ.
Pouches are typically painless and pain may signify ulceration of the mucosa or carcinoma within the pouch.
The pouch may be made to fill with a Valsalva manoeuvre at the time of investigation causing contrast to track into the pouch.

This is protrusion of the mucosa through the Killian's dehiscence, a weak area of the posterior pharyngeal wall between the oblique fibres of thyropharyngeus and the transverse fibres of the cricopharyngeus at the lower end of the inferior constrictor muscle.
Further enlargement of the pouch will cause a gurgling noises in the neck with swallowing and the patients may present with a swelling in the neck.
The swelling is usually on the left side of the neck because as the pouch enlarges the resistance of the vertebral column behind cause it to turn laterally to the left.

Interventional procedures consultation document - endoscopic stapling of pharyngeal pouch
Pharyngeal pouch, which is also known as Zenker's diverticulum, occurs when a piece of the pharyneal lining herniates through the muscles of the pharyngeal wall.
A pharyngeal pouch may cause difficulty in swallowing or cough, and sometimes causes respiratory problems because of aspiration of the pouch contents into the lungs.
Endoscopic stapling of pharyngeal pouch involves stapling of the opening of the pharyngeal pouch using a specially designed endoscope, while the patient is under general anaesthetic.

Functional evidence for a single endodermal origin for the thymic epithelium - Nature Immunology
Formation of a third pharyngeal cleft derivative, the cervical sinus, was noted at E10.5−E11, and this subsequently underwent complex proliferation and rotation, such that ectodermally derived cells became attached to the third pharyngeal pouch at around E11 and covered the third pouch endoderm by E11.5−E12.0.
This suggests that the pharyngeal pouch and cleft separate after contact and that both this process and release of the thymic primordium from the pharynx entail an apoptotic mechanism.

Pharyngeal pouch (Zenker’s diverticulum) is a pulsion diverticulum which originates from Killian’s dehiscence between the cricopharyngeus and thyropharyngeus muscles.
For the majority of patients, however, excision of the pouch through an incision in the neck is the treatment of choice, usually combined with a cricopharyngeal myotomy.
The pouch was transacted flush with the stapler according to the manufacturers’ instructions (Operation of the Proximate Reloadable Linear Stapler, Ethicon Ltd.) and the stapler removed.

Pharyngeal pouch (Zenker's diverticulum) is a false diverticulum of the pharyngeal mucosa that occurs just above the cricopharyngeal sphincter, in an area of weakness between the cricopharyngeus and the inferior pharyngeal constrictor muscle (figure 2).
The estimated prevalence of this condition is 1 in 100,000, mainly occurring in older subjects.
The pouch probably arises as a result of relative obstruction caused by muscular incoordination of the cricopharyngeal sphincter.

Endoscopic stapling to treat pharyngeal pouch is a relatively new technique with the potential to reduce the morbidity associated with the open approach for pharyngeal pouch.
The aim of the present study was to determine the outcome associated with endoscopic stapling of pharyngeal pouch in an Australian setting.
All patients admitted for endoscopic stapling for a pharyngeal pouch between 1998 and 2002 by surgeons from the Adelaide and Flinders Universities were identified, and their medical records were reviewed for clinical and operative details.
