Burning Void--Plot Twists for Game Masters
Plot twists are probably the major way to keep roleplaying plots fresh, exciting, and unpredictable.
Also remember to go back and fill in any plot holes the new twists create--this is why it's a good idea to pick your plot twists early on when creating your adventure.
The plot the characters think they're working on can turn out to be a different plot entirely, or you can add a new plot onto the group's responsibilities.

Plot twists are also unexpected in that they reveal as true an option you had not even considered.
Since plot twists can not come from the rules (for reasons I think are obvious, but which I am still hoping to explore in a later article), one of the audience members must be the source.
The end result of all this is that good plot twists come from good procedures for generating material combined with a set of skills that are not often associated with roleplaying.

Burning Void--Plot Twists for Stargate SG-1 Adventures
I posted an article on plot twists that includes resources (web sites and books) as well as a genericized randomized chart of plot twists for those times when you need some inspiration.
I also think a good list of plot twists should be ever-changing--add new twists you think up, remove ones that are getting stale, lower the probability of a twist you've used once but could potentially still get some mileage out of, and so on.
Many plot twists also represent a range of possibilities, so you might simply cross off that specific possibility but leave the generic twist on the list.

Dan Heller's Photos/Pictures of Movie Plot Twists: An Analysis
There seems to be a disturbing trend in the "surprise-ending" tactics used by certain mystery/thriller movie-makers these days: they build a story line around the psychological objectives of the main characters, but then surprise the audience with a logically-acceptable plot twist that abandons (or violates) the characters' original objectives.
While movie audiences are filling theaters and movie rentals houses to watch suspense films for the fun-filled about-face turn-around at the end, the method that the filmmakers are using to tickle the audience is based on a short-sighted formula that may not have legs if not done more carefully.
Here, Scott's character loses his family in a car accident, and when he moves into a house that happens to be haunted, he slowly discovers its ghost is a young boy who could identify with Scott's character's plight, and the two help each other with their objectives.

Playing it Straight With Plot Twists
While it is possible to treat your plot mechanically on some levels, plot twists have to arise out of your characters's lives in order to be effective.
A plot twist that simply changes the direction in which a character's life is going can be effective, but the really moving plot twists also change the way the character looks at the world.
But perhaps the easiest approach is to treat your plot twists as if they were the revelation of the killer at the end of a mystery.

 Oliver Twist
The illustrations for Oliver Twist by George Cruikshank (1792-1878) are thought by many to be some of the finest for any Dickens novel.
In 1872, two years after Dickens' death, Cruikshank claimed that the plot and many of the characters from Oliver Twist had been his idea, a claim which Dickens' friend and biographer, John Forster, vehemently denied.

Twist ending - Wikipedia, the free encyclopedia
A "twist ending" or "surprise ending" is an unexpected conclusion or climax to a work of fiction, which may contain an irony, or cause the audience to reevaluate the rest of the story.
A misdirection is similar in meaning to the red herring: the writer uses both to distract the protagonist, and by extension the reader, away from the correct answer.
However, she has retained the overall plot structure centering around an important mystery, throughout the book planting subtle (often completely innocuous) clues, which she reveals (always in a twist ending) at or after the climax.
