This is something that I have experienced in which there are indications of erosion of the PVC pipe around the nozzle plug.
In a bench test in which I cast a plug of concrete into a short length of pipe, I was able to break the plug loose with finger pressure.
One hole is the nozzle and the other end is the touch hole.
