At its mouth, the river is essentially an arm of Chesapeake Bay, whereas the upper river flows through a series of relatively inaccessible wetlands, called the Pocomoke River Swamp, populated by loblolly pine and baldcypress.
The river is the easternmost river that flows into Chesapeake Bay and is considered to be one of the deepest rivers for its width in North America.
In 1635 the mouth of the river was the scene of the first recorded battle in North America between Englishmen.

Settled in the 1600's on the banks of the Great Pocomoke River, Pocomoke City was originally known as Steven's Ferry after the ferry that crossed the Pocomoke River transporting tobacco and other goods.
By the early 1900's ship building was the largest industry in Pocomoke with furs, whiskey and tobacco finding their way via the Pocomoke River (the deepest and most scenic river in Maryland) to ports in Baltimore, New York and Philadelphia.
Pocomoke City held a franchise in the Eastern Shore Baseball League.

According to American Rivers, selections were based on the seriousness and imminence of the threat; the likelihood that major action during the coming year could intensify or lessen the threat; the regional and national significance of the river; geographical diversity; and the range of threats representative of problems facing rivers nationwide.

The Pocomoke River — the scene of fish kills two years ago — has levels of dissolved metals and steroids that in some cases are “orders of magnitude” higher than those typically seen in Chesapeake tributaries, according to a new study by investigators at the U.S. Geological Survey and George Mason University.
Not all of the steroid compounds in the river have been identified, but several are in a class of compounds called estradiols, which have the potential to be “hormone disrupters” — substances that mimic some hormones in animals, affecting growth and reproduction.
But the steroids in the river could be of more concern, Foster said, because they are in the water column where they are directly available to aquatic life, and because they are found at relatively high concentrations.

Consequently, she is concerned for the health of fishermen and people who eat diseased fish and has campaigned against the urban and agricultural developments that are nourishing the river waters of North Carolina and allowing P. piscicida and its deadly relatives to flourish.
Pocomoke River fishermen reported finding lesions (sores) on many of the fish they caught in the river, first in October 1986, and again this April.

The sun was shining bright and although the narrow river was relatively protected from the high wind, there were a few portions where we did paddle into the wind.
On the stretches of the river where we paddled into the wind, we struggled to maintain a paddling speed of 2 miles per hour and worked hard at that.

The reason is that the lower Pocomoke is brackish and the upper Pocomoke is freshwater, and Pfiesteria "just doesn't grow very well without a little bit of salt," according to JoAnn Burkholder, a North Carolina State University researcher who first identified the microorganism and has been studying it since 1991.

On Aug. 29, 1997, MD and VA officials closed a 7-mile stretch of the lower Pocomoke River indefinitely to fishing, swimming, and recreational boating to limit human exposure to Pfiesteria toxin that medical researchers from Johns Hopkins Hospital and the Univ. of MD Medical Center believe has caused illness in at least 7 people.
On Sept. 2, 1997, a scientific advisory committee was scheduled to meet in Annapolis, MD to examine datafrom the Pocomoke River.
MD officials were also reported that the Nanticoke River is of concern since it is similar to the Pocomoke River, and that sampling was being conducted onthe Patuxent River.

The study has shown that Pocomoke River tributaries in the central part of the watershed exhibit turbidity even during dry years (Figure 8).
When this water emerges at the surface or is discharged into the river system it rapidly oxidizes to an amorphous particulate iron oxyhydroxide which in time crystallizes to goethite.
Whatever the mix of materials that cause the turbidity, they are authigenic in the rivers and are not contributed by runoff.

Arriving back at the Pocomoke River Canoe Company the outfitter staff assisted our flotilla onto the floating dock, a magnanimous gesture they would repeat as various Duckhead trips concluded at their establishment throughout the weekend.
Rounding a bend in the river we discovered the true source of this phenomenon to be a gigantic floodlight in the front yard of the only house along this stretch; an enormous runway-style light pointed blindingly downriver.
