 Ventricular Tachyarrhythmias
Polymorphic ventricular tachycardia typically is irregular in rate and rhythm and has varying shapes or morphologies on the ECG.
Ventricular tachycardia may give rise to symptoms such as palpitations, shortness of breath, or lightheadedness, depending upon the rate of the arrhythmia, its duration, and the underlying heart disease.
Ventricular arrhythmias and syncope are often provoked by increases in heart rate in response to exercise or emotion.

 s000626a - Torsades de Points
Abstract: Torsades de pointes is a polymorphic form of ventricular tachycardia that is usually associated with prolongation of the QT interval.
We report a case of QT prolongation and polymorphic ventricular tachycardia (torsades de pointes) that was precipitated by the intravenous administration of erythromycin.
This ventricular tachyarrhythmia, characterized by rapid (200-250 beats/min) and irregular paroxysms and progressively varying QRS amplitude and polarity, occurred in the absence of electrolyte disturbance, antiarrhythmic drug therapy or acute ischemia.

 Welcome to Ventricular Tachycardia
Ventricular tachycardia is " a tachycardia where the origin of the arrhythmia is within the ventricles".
That is, the tachycardia does not require the participation of the atrium or AV node and His bundle although the latter structures may be activated "passively" by retrograde conduction.
Ventricular tachycardia related to acute ischemia or that is extremely rapid is frequently polymorphic, The probability of the presence of VT in a patient with cardiac disease increases in general with the severity of cardiac disease and is most likely to be fatal in patients with impaired ventricular function.

Amiodarone has been approved for treatment of patients with ventricular and supraventricular tachyarrhythmias.1—3 Previous reports have found that it is effective for the conversion of atrial fibrillation and subsequent maintenance of sinus rhythm.2—6 In this report, we describe a case of atrial fibrillation that converted with amiodarone.
Although that is a well-documented complication of this drug, this case report underscores the need to remain vigilant for serious and life-threatening arrhythmias in patients who are exposed to amiodarone.

The development of polymorphic ventricular tachycardia in ibutilide patients is significantly correlated with female gender, non-White race, presence of heart failure and lower pulse rate.
Torsades de pointes, a polymorphic ventricular tachycardia that develops in the setting of a prolonged QT interval, may occur because of the direct effect of ibutilide on cardiac repolarization, however, ibutilide can also cause polymorphic ventricular tachycardia in the absence of excessive prolongation of the QT interval.
Ibutilide infusion should be stopped as soon as the presenting arrhythmia is terminated or in the event of sustained or nonsustained ventricular tachycardia, or marked prolongation of QT or QTc.

Brugada Sydrome - Electrophysiologic and Hemodynamic Findings
All published studies agree on the inducibility of polymorphic ventricular tachycardia by programmed electrical stimulation in symptomatic patients.
The induced arrhythmia is sustained in practically all cases, results in hemodynamic collapse and has to be terminated by an external DC shock.
It can be criticized that polymorphic ventricular tachycardia or ventricular fibrillation induced by programmed stimulation is a non-specific finding, because these arrhythmias can sometimes be induced in patients with a normal heart.

eMedicine - Torsades de Pointes : Article Excerpt by: Syed Nazeer, MD
Background: Torsades de pointes, literally meaning twisting of points, is a distinctive form of polymorphic ventricular tachycardia (VT) characterized by a gradual change in the amplitude and twisting of the QRS complexes around the isoelectric line.
Torsades de pointes (torsades) is associated with a prolonged QT interval, which may be congenital or acquired.
It usually terminates spontaneously but frequently recurs and may degenerate into sustained VT and ventricular fibrillation.
