Portkeys are first mentioned in Harry Potter and the Goblet of Fire.
This is why Moody cast the portkey spell on the Tri-Wizard Cup since, it was originally intended as a portkey all along by the tournament officials for the winner of the cup to exit the maze without having to fight past the other champions and challenges of the maze again.
It would explain why the portkey took Harry back to Hogwarts and out of the maze when he touched the Cup the second time instead of back into the maze at Hogwarts (or not functioned at all like regular one time portkeys).

I admit that arrival via portkey is somewhat disorienting and that it's not practical for all the DE's to show up at once.
Also, if Crouch had really been that heavily involved in a plan involving use of the Portkey as a means of allowing the DEs to attack Hogwarts, then I would rather expect for him to be a bit more interested in learning from Harry why the plan didn't work.
If he dropped the Portkey and someone else picked it up, he would be stuck at Hogwarts, not exactly a glorious climax to a triumphant return.

It means that the first time that the Portkey is touched, it will follow its new orders ("take user to graveyard"), while the second time it is touched, it reverts to its original programming ("take user to periphery of maze.")
That's certainly a possibility, although I think that I still prefer the idea that the plan was originally to use the Portkey's secondary destination, either to descend on Hogwarts or simply to send Harry's corpse back as a nice little message for the wizarding world.

I'm going to guess that a) portkeys can be one way or two way and that the cup was a two way portkey that simply brought Harry back.
If portkeys always return to their point of origin, then the adult shadows would have known this.
He says to Harry, "you must get to the Portkey, it will return you to Hogwarts." So, I see it as either he knew that Portkeys can be two-way - perhaps in case you forget something and need to get back, or that Lily (I like this idea by the way) charmed it somehow.

The limit appears to be that you can't bring any other living wizard with you (everybody having to touch the Portkey in GoF).
Apart from that I agree entirely - we have (IMO) clear evidence that magic items (and to some degree spells) are capable of at least limited mind-reading.
