With Pure O, the compulsions manifest as unseen mental rituals, but they are compulsions nonetheless, which is why the term "pure obsessional" is somewhat imprecise.
With ongoing progress in mental health research, including fine-tuning in the field of anxiety, diagnoses of pure obsessional OCD have increased in recent years.
A person with Pure O experiences periods of intense rumination that are triggered by intrusive or unwanted thoughts, sometimes called "spikes." Spike traits vary widely by individual, dictated by personal makeup and circumstance.

Steven Phillipson - Wikipedia, the free encyclopedia
Steven Phillipson, Ph.D., is an American cognitive-behavioral psychologist in the field of obsessive compulsive disorder (OCD) and a specialist in the emerging field of Pure Obsessional OCD ("Pure O").
Pure O differs from traditional OCD in that it manifests solely in the mind, rather than through the compulsive rituals (hand-washing, checking, etc.) associated with the more well-known form of the disorder.
Phillipson's OCD treatment focuses on the psychological principles of extinction and habituation, emphasizing the role of the brain's amygdala in the experience of anxiety.

Obsessive Compulsive Disorder, or OCD, is an anxiety disorder in which a person feels compelled to engage in certain physical or mental acts (“rituals” or compulsions) in order to relieve their anxiety connected with intrusive thoughts (obsessions).
OCD with mental rituals only is sometimes referred to as Pure Obsessional OCD or “Pure O”, as both the obsession and the compulsion are mental acts rather than physical acts.
One advantage of psychological therapy is that, because during therapy a person learns techniques to overcome their anxieties and compulsions, they can continue to apply these techniques after therapy has finished.

I also have had more of a problem with my girlfriend, whom I am starting to treat as an extension of my own head, rambling on to her, exploring every sub-sub-sub angle of a situation, but mostly asking her for reassurance.
I have never known I had OCD; I on occasion have physical compulsions due to my thinking but mostly everything is in my head, so I have always thought it was my own problem that I had to deal with.
I am just looking to find more people with pure obsessional OCD- even though I have my girlfriend, I know no one and nothing about OCD besides my own problems, and I would just like to know other people such as myself are out there (corny as that might sound, hehe).

New York based Phillipson in fact classifies “Pure O” as a psychological condition rather than a mental illness or disorder.
For instance: Genuinely bad news — say, word of a friend’s hospitalization — may not trump or even penetrate the anxiety felt by an OCD sufferer who is ruminating on even a seemingly innocuous matter.
I can relate to Pure O… I had never heard of it defined as that before, but that very well puts a finger on exactly what my old anxiety experiences were like.
