In August 1990, a driver died and 35 passengers were injured in a two-train crash at Safford station.
Since 1988 there have been six fatal crashes on Britain's railways, including that at Southall in 1997 when seven people died and around 150 were injured as an InterCity train from South Wales ploughed into a freight train in almost exactly the same spot as Tuesday's Paddington crash.
Investigations into the Paddington crash are to concentrate on the behaviour of the Thames turbo train "following reports that it passed a signal at danger", it was announced Wednesday.

The rail depicted, allowing for a degree of artistic licence to use its cross-section for a sketch-map of the proposed routes, is clearly flat-bottomed.
The Pendleton Station area has frequent bus services, Salford Crescent station is half a mile away and anyway the walk would be beneficial to the health of those who sit down all day at work and who sit on a bus or train when travelling thereto and -from as well.
The return journey by coach to the station reveals that, with it being positioned to the east of the dual carriageway, it is necessary to head by road back to the main line itself, pass over it, negotiate a roundabout and recross the line in order to return south to Heathrow Junction.
