If the President of the Quorum of the Twelve Apostles is one of the Counselors in the First Presidency, he retains that title, but is not numbered among the Twelve, and the most senior Apostle not in the First Presidency is named Acting President of the Quorum of the Twelve.
Became senior Apostle in 1839, ordained President of the Twelve in 1840, became President of the Church in 1847.

In Mormonism, the Quorum of the Twelve Apostles (also known as the Quorum of the Twelve, the Council of the Twelve, or the Twelve) is one of the governing bodies of church hierarchy in many Latter Day Saint denominations.
Members of the Quorum of the Twelve Apostles are considered apostles, with a special calling to be evangelical ambassadors and witnesses of Jesus Christ.
The Quorum was organized in 1835 and designated as a body of "traveling councilors" with jurisdiction outside areas where the church was formally organized, equal in authority to the First Presidency as well as to the Seventy, the standing Presiding High Council, and the High Councils of the various Stakes (Doctrine and Covenants 107:25-27, 36-37).

Brigham Young, then President of the Quorum of the Twelve Apostles, presented the proposition that the Twelve, ordained apostles who held all the keys necessary to govern the Church, should be sustained as the authorized leaders in the absence of Joseph Smith.
Inherent in the Twelve´s proposal was the assumption that, although the Quorum of the Twelve Apostles had equal authority and power with the Quorum of the First Presidency, as long as the First Presidency was intact and functioning, they, and not the Twelve, possess the necessary jurisdiction to govern the Church.

QUESTION:  In recent church history, we had an instance where the President of the Quorum of the 12 and the Acting President of the Quorum of the 12 were both serving in the Quorum of the 12 (the President of the Quorum of the 12 was not serving in the 1st Presidency).
He was first set apart as Acting President of the Quorum of the Twelve on June 5, 1994 when President Hunter called Elders Hinckley and Monson to be his counselors in the First Presidency.
Thus, Elder Packer, the next most senior member of the Quorum, was again set apart as Acting President of the Quorum of the 12 (Elder Monson served as President of the Quorum of Twelve and as 1st Counselor in the First Presidency).
