Railroad Switch Safety Demonstration Project Begins Testing - Technology - RedOrbit
The test involves the installation of wireless communication devices at 49 switches along a 174-mile section of non-signalled BNSF Railway track between Tulsa and Avard, OK. Train dispatchers at an operations center in Fort Worth, TX will monitor the devices to identify when hand-operated switches are set in the wrong position.
If a switch is misaligned, the dispatcher directs a train to stop until railroad crews in the field confirm it is safe to proceed.
In October, an Emergency Order was issued directing railroads to immediately re-instruct employees on how to operate these switches and to communicate whenever a switch is operated.

Railroad switch - Wikipedia, the free encyclopedia
The position of the switch is usually changed electrically on main lines and controlled from a remote control center, or an interlocking tower/signal box, from where staff also alter semaphores or light signals correspondingly.
This is generally referred to as a switch's "number." For example, on a "number 12" switch, the rails are a distance of one foot (1'-0") apart at distance of twelve feet (12'-0") from the center of the frog.
A switch motor is an electric or pneumatic mechanism that aligns the points with one of the diverging routes.

Railroad switch design and failure mode analysis - Ttci R&D - Transportation Technology Center Inc Railway Track and ...
Switch study sponsored by TTCI finds that switch maintenance, not a better switch design, is key to maximum product service life and reduced derailments due to broken, worn or damaged switches.
Accidents caused by switch operation (i.e., human error) and dragging equipment or "one wheel off" accidents, where the train has already derailed but "piles up" at the switch, may be difficult to eliminate with a better-designed switch.
While switch performance has improved since the 1970s and the rate of switch-related accidents has decreased, they are still routinely in the top five track causes in the Federal Railroad Administration's accident database.
