Back to episode four, and in line with the theme of marriage, the storyline is based around Johnny Sack's devotion to his wife Ginnie, and how upset he gets when Ralph tells a joke about her weight.
When Tony confronts Ralph about the incident the fight that ensues ends with Ralph dead, and Tony calls his nephew Christopher Moltisanti to help him dispose of the body.

I thought they way they played it out was to leave you wondering if Ralph actually did it, or if he was just using it to ride Tony, since Tony was obviously so sensitive about it.
But I think Tony's love of animals, hatred of Ralph, and most importantly...Tony's perception of Ralph's guilt due to Ralph's "It's just a horse and I've got more impotant things on my mind" attitude in the face of his son's recent injury is what led to the events in the episode.
