In a CRT, the scan rate is controlled by the vertical sync signal generated by the video controller, ordering the monitor to position the electron gun at the upper left corner of the raster, ready to paint another frame.
The refresh rate can be calculated from the horizontal scan rate by dividing by the number of horizontal lines and multiplying the result by 0.95 (since about 5% of the time it takes to scan the screen is spent moving the electron gun back to the top).

I guess to produce a frame rate of 300fps, the monitor would need a refresh rate of about 0.3ms or 300Hz.
If I remember correctly, having a higher refresh rate does help on newer lcd monitors because they have only begun to have fast enough displays to update the information along with the refresh rate.
I do notice that It helps me to put my refresh rate higher on my 16ms panel because for some reason I get the feeling that there is less lag, but it could be the placebo effect.

My thought on the refresh rate of a LCD is that the LCD has a fixed clock, probably at 60 hz, so no matter how much info you send it ie 75 hz instead of 60hz it will still only show at 60hz due to the LCD display's clock.
While its true that LCD's dont refresh like CRT's do the refresh rate does determine the rate at which updates are sent from the video card to the LCD.
The DVI port on current video cards has a limiting bandwidth issue and this is why you can select higher refresh rates at lower resolutions but at higher resolutions like 1680x1050 you can't choose anything higher then 60Hz.

I would like to know how to run at higher resolution when I am not playing CS with a lower refresh rate (since it doesnt support high refresh rate + high rest) and then run high refresh rate when I play CS at 800x600..
look for a problem called reforce and it will force your refresh rate to be what you set it.
Try moving an open window up and down in that refresh rate and you can see it skip a little.

you were insinuating that the refresh rate is always fixed and is either 60 Hz (all, except Pioneer) or 72 Hz (Pioneer).
There's not even a technical reason why a fixed internal refresh rate would have to be the case -- it's not as if plasmas have an electron beam that scans at a pre-determined and non-adaptable rate (like non-multisync CRT tubes).
For this to happen the panel with its progressive scan processing has to be operating equivalent to a 70hz or better refresh rate.

Note that running at higher refresh rates is all very nice, but it means the game has much less time between frames to do everything necessary and could potentially drop to half the rate it was initially (not good).
For example, if the refresh rate is 75 Hz, you'd get perfect smooth movement at 15 fps and 25 fps without any work at all (well, in theory, at least).
It seems less than ideal to me to be running a game engine at a lower framerate than your monitors refresh rate, as things are not running as smooth as they could be.
